They must provide nursing care to patients, support relatives and document for municipalities and regions, and breaks are often pushed aside during hectic shifts. It does not come naturally to slow down and take a break when there is still so much left to do. It was easier back when we had smoking breaks. Back then you could \u201cneed\u201d a cigarette without anyone questioning the necessity of the break. And yes\u2026 smoking breaks were physically unhealthy, but we argue that they were important for our minds, our professionalism and our resilience in the profession.<\/p>\n<p class=\"my-2 [&amp;+p]:mt-4 [&amp;_strong:has(+br)]:inline-block [&amp;_strong:has(+br)]:pb-2\"><strong>Keywords:<\/strong> Nurses\u2019 breaks, workload, well\u2011being, resilience, smoking culture, nursing work environment<\/p>\n<\/div>\n<p><strong>Introduktion<br \/>\n<\/strong><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">H\u00f8je f\u00f8lelsesm\u00e6ssige krav i sygepleje<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I Danmark er arbejdsgiver forpligtet til at sikre at ansatte ikke bliver syge af at g\u00e5 p\u00e5 arbejde. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I 2023 udgav Arbejdstilsynet en vejledning om arbejdsgiverens vigtigste pligter i forhold til at arbejde forebyggende med h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger i arbejdet med mennesker. Vejledningen er d\u00e6kkende for mange forskellige erhverv, herunder ogs\u00e5 omsorgs- og sundhedsfag.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">H\u00f8je f\u00f8lelsesm\u00e6ssige krav defineres som arbejde, der indeb\u00e6rer direkte eller indirekte kontakt med mennesker, hvor man skal kunne s\u00e6tte sig ind i, rumme og h\u00e5ndtere den andens t\u00e6nkning, f\u00f8lelser og\/eller adf\u00e6rd samtidig med at man h\u00e5ndterer egne tanker og f\u00f8lelser og tilpasser kommunikation til den, der arbejdes med (1).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Unders\u00f8gelsen <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">\u201dArbejdsmilj\u00f8 og helbred\u201d<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> fra 2018 viser, at konkrete arbejdsopgaver s\u00e5som at skulle forholde sig til mennesker i sv\u00e6re f\u00f8lelsesm\u00e6ssige situationer, fx patienter der er ramt af alvorlig sygdom, ulykke, sorg, krise eller sociale problemer h\u00e6nger sammen med en h\u00f8jere risiko for langtidsygdomsfrav\u00e6r, og if\u00f8lge unders\u00f8gelsen ligger sygeplejersker over gennemsnittet i forhold til h\u00f8je f\u00f8lelsesm\u00e6ssige krav i arbejdet (2).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Sunde pauser uden r\u00f8g<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I 2013 gennemf\u00f8rte K\u00f8benhavns Lufthavn, i samarbejde med Kr\u00e6ftens Bek\u00e6mpelse, en stor unders\u00f8gelse, som de kaldte <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">\u201dEt godt liv med sunde pauser\u201d<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">. Baggrunden for projektet var et totalt rygestop, som fratog medarbejderne muligheden for rygepauser.<br \/>\n<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Som en del af projektet blev der udf\u00f8rt fokusgruppeinterviews, der konkluderede et behov for flere forskellige elementer og tiltag til pauser, nu hvor rygepauser ikke l\u00e6ngere var mulige. Noget af det, som var et gennemg\u00e5ende behov fra medarbejderne, var et \u00f8nske om muligheden for at se og anerkende hinanden samt at skabe forst\u00e5else for forskellige behov p\u00e5 tv\u00e6rs af kulturer. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">En af anbefalinger fra projektet l\u00f8d, at det er vigtigt, at der skabes legitimitet fra ledelsens side og at det kommunikeres klart ud til medarbejderne, at det er i orden at holde de n\u00f8dvendige pauser og sm\u00e5 breaks i l\u00f8bet af arbejdsdagen (3).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Mental luft til lungerne<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Rygepauser var ogs\u00e5 et issue for os, da vi som unge og nyuddannede sygeplejersker startede p\u00e5 psykiatrisk afdeling. Her var vagterne altid en kamp mod r\u00f8gskyerne fra cigaretterne. M\u00f8bler, t\u00f8j og alt andet inventar stank som et gammelt v\u00e6rtshus. Vi r\u00f8g ikke selv og f\u00f8lte n\u00e6rmest ubehag ved at f\u00e6rdes i r\u00f8gskyerne.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Vi opdagede dog hurtigt at gode snakke, kontakten med patienterne; den sp\u00e6de relationsopbyggende og den skr\u00f8belige relationsreparerende indsats fandt sted over en sm\u00f8g.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Og det var ikke kun patienterne, der brugte cigaretterne som noget at tale hen over!<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Vores rygende kolleger gjorde det ogs\u00e5. Mindst hver tredje time samlede de sig p\u00e5 terrassen udenfor. Her r\u00f8g de og snakkede sammen. Til at begynde med t\u00e6nkte vi ikke n\u00e6rmere over det. Efter lidt tid begyndte det at irritere os. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Sammenlagt p\u00e5 en hel vagt, var det n\u00e6sten 1 time, at rygerne havde pause, og vi f\u00f8lte os snydt for pauserne og f\u00e6llesskabet. I en periode overvejede vi, om vi skulle begynde at ryge for at kunne v\u00e6re med i rygepauserne. I stedet besluttede vi at deltage i rygepauserne uden at ryge med tanken om at <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">\u201dn\u00e5r de kunne holde pause, s\u00e5 kunne vi ogs\u00e5\u201d<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">.<\/span><\/p>\n<p><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Hurtigt opdagede vi, at rygepauser ikke blot var samv\u00e6r og hyggesnak. Her blev talt om det som var sv\u00e6rt i arbejdet, og gode r\u00e5d blev givet til dem, som havde brug for det.<br \/>\n<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Der var holdninger og faglige teorier, som blev diskuteret. Der blev talt om det, som ber\u00f8rte os, fx situationer hvor man havde oplevet sig utryg eller f\u00f8lelsesm\u00e6ssigt belastet. Der var lydh\u00f8rhed og anerkendelse af det fortalte.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Rygepauserne var et helle i arbejdet, en n\u00f8dvendig \u2013 om end rygende usund \u2013 pause, som gav mental luft til lungerne til at v\u00e6re i de h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger p\u00e5 psykiatrisk afdeling. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Set retrospektivt brugte vi pauserne ligesom medarbejderne i K\u00f8benhavns lufthavn, og netop indholdet i rygepauserne var vigtige for vores engagement, dannelse og faglighed som psykiatriske sygeplejersker. Krisepsykolog Rikke H\u00f8gsted og forfatter til bogen Grundbog i belastningspsykologi skriver:<\/span><\/p>\n<div class=\"farve-boks\" style=\"background-color: #b8e1c0; padding: 16px 32px; border-radius: 24px; color: black; margin-bottom: 32px;\">\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><em>&#8220;at v\u00e6re del af en velfungerende arbejdsgruppe er noget af det mest forebyggende der findes i forhold til at h\u00e5ndtere psykiske belastninger og undg\u00e5 mental slagside&#8221;<\/em> (4)<br \/>\n<\/span><\/p>\n<\/div>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">H\u00f8gsted uddyber dette med, at kollegaer s\u00e6rligt kan st\u00f8tte hinanden, fordi de gennem levede erfaringer kan spejle hinanden (4). <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I bogen beskriver H\u00f8gsted (2019), hvorledes vi, gennem vores spejlneuroner og empatisk resonans, l\u00e6ser andre og indlever os i deres gl\u00e6der og sorger. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">If\u00f8lge den tyske sociolog Hartmut Rosa sker der i det moderne vestlige samfund en konstant stigning i tempo og effektivisering. P\u00e5 trods af markante teknologiske fremskridt er det paradoksalt, at tidsmangel er en udbredt f\u00f8lelse i samfundet. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">B\u00e5de H\u00f8gsted og Rosa ser den gensidige, empatiske f\u00f8lelsesm\u00e6ssig forbindelse mellem mennesker som n\u00f8glen til at fremme trivsel og personlig v\u00e6kst.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Hartmut Rosa siger om resonans:<\/span><\/p>\n<div class=\"farve-boks\" style=\"background-color: #b8e1c0; padding: 16px 32px; border-radius: 24px; color: black; margin-bottom: 32px;\">\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><em><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&#8220;Det gode m\u00f8de er kendetegnet ved, at det altid g\u00f8r en forskel. I det gode m\u00f8de bliver din tilegnelse af verden derfor \u2019transformativ\u2019 i stedet for \u2019instrumentel\u2019. Men netop den <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">tilegnelse er selvf\u00f8lgelig truet i vores h\u00f8jhastighedssamfund, for den er jo langt mere tidskr\u00e6vende. Dog, n\u00e5r du virkelig m\u00f8der noget eller nogen, der g\u00f8r en forskel for dig og dit forhold til verden, s\u00e5 sker der det, jeg kalder \u2019resonans\u2019. Forst\u00e5et som den t\u00e6tte kontakt, der forandrer dig, ber\u00f8rer dig, og hvor du ikke bare selv tager imod, men ogs\u00e5 selv r\u00e6kker ud. Hvor du ikke l\u00e6ngere f\u00f8ler dig fremmedgjort. <\/span><\/em><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><em>S\u00e5 i min teori er resonans alts\u00e5 det modsatte af fremmedg\u00f8relse. Eller svaret p\u00e5 problemet&#8221;<\/em> <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">(5).<\/span><\/p>\n<\/div>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Effektivisering ind, pauser ud<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Med det totale offentlige rygestop p\u00e5 arbejdspladser i 2012\/13 forsvandt rygepauserne. Samtidig var der i sundhedsv\u00e6senet stort fokus p\u00e5 kvalitetsarbejde, dokumentation, standarder og retningslinjer. New Public Management, Lean-modeller og forbedringsarbejde blev introduceret og \u201cspild\u201d blev sk\u00e5ret fra. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Hartmut Rosa kalder vores samfund for accelerationssamfundet. Alt sammen i et s\u00e5dant omfang, s\u00e5 sygeplejersken sj\u00e6ldent har tid til pauser. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Arbejdstilsynet skriver i deres vejledning om h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger, at det er arbejdsgiverens opgave at forebygge h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger i arbejdet med mennesker, hvorfor arbejdet skal planl\u00e6gges, tilrettel\u00e6gges og udf\u00f8res sikkerheds- og sundhedsm\u00e6ssigt fuldt forsvarligt i forhold til de h\u00f8je f\u00f8lelsesm\u00e6ssige krav, der m\u00e5 v\u00e6re i arbejdet (1).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Rikke H\u00f8gsted (2019) mener ligeledes, at det er organisationens og ledelsens ansvar at f\u00e5 skabt en mental b\u00e6redygtig arbejdsplads, hvor der er balance mellem udnyttelse og beskyttelse af de ansattes ressourcer. H\u00f8gsted foresl\u00e5r bl.a. at der skabes rum og stund for at kollegaer kan dele oplevelser og give de st\u00e6rke indtryk et udtryk (4).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Da rygepauserne gik ud og effektiviseringen kom ind, blev mulighederne for gode rettidige samtaler med kollegaerne, n\u00e5r belastningerne blev h\u00f8je, indsn\u00e6vret. De blev skubbet til supervisionerne, som mange steder ogs\u00e5 var blevet besk\u00e5ret til at v\u00e6re m\u00e5nedligt eller endda endnu mindre. Eller de kunne dr\u00f8ftes p\u00e5 konferencerne, hvis det ikke var fordi, at tiden der i forvejen var presset nok og ofte en kamp at n\u00e5 konferenceprogrammet p\u00e5 ordentlig og reflekteret vis. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Frokostpauserne kunne ogs\u00e5 v\u00e6re en mulighed. Dog har mange arbejdssteder en kultur, hvor der ikke tales om arbejde i frokostpausen. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Rygepauserne var alts\u00e5 s\u00e6rligt gode fordi, de blev holdt, n\u00e5r der var behov for det. N\u00e5r man havde brug for en pause til at tr\u00e6kke vejret (igennem en cigaret).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Her havde man mulighed for at t\u00e6nke noget om det, man lige havde erfaret. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Man havde mulighed for at g\u00f8re sig tanker om det, sig selv, patienten og relationen imellem. Man kunne f\u00e5 spejling og sparring af kollegaer. Alt sammen inden man afsluttede pausen for at g\u00e5 tilbage til arbejdet. Nu med nye indsigter og deraf ogs\u00e5 nye handlemuligheder.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Sindet som redskab<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I vores arbejde som sygeplejersker, har vi brug for pauser. Ikke kun til at spise og g\u00e5 p\u00e5 toilettet. Vi har brug for pauser, fordi vi bruger os selv som redskab i sygeplejen.<br \/>\n<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Som unge psykiatriske sygeplejersker l\u00e6rte vi, at en kirurg har sin skalpel og vi havde vores sind (hvorigennem vi skulle s\u00f8ge at forst\u00e5 patienten). Rikke H\u00f8gsted underst\u00f8tter dette og skriver, at:<\/span><\/p>\n<div class=\"farve-boks\" style=\"background-color: #b8e1c0; padding: 16px 32px; border-radius: 24px; color: black; margin-bottom: 32px;\">\n<p><em><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&#8220;fagpersonen stiller sig selv til r\u00e5dighed og bruger sin indlevelses-evne for at opn\u00e5 den andens tillid, s\u00e5 der kan laves en faglig god analyse og handlingsplan&#8221;<\/span><\/em><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> (4).<\/span><\/p>\n<\/div>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Sygeplejersken skal alts\u00e5 fungere som en slags resonanskasse, der opfanger patientens kommunikation og r\u00e6sonnerer over den, inden hun handler meningsfuldt og hj\u00e6lpende r\u00e6sonnerer tilbage til patienten. Sygeplejersken skal kunne koble f\u00f8lelser og fornuft. Hun skal t\u00e6nke noget om det hun m\u00e6rker og m\u00e6rke noget om det hun t\u00e6nker. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Med andre ord; sygeplejersken skal mentalisere (7). Kirurgens skalpel skal v\u00e6re skarp. Sygeplejerskens sind skal v\u00e6re mentaliserende. Til det er tid og pauser n\u00f8dvendige.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Mentalisering bidrager til resiliens<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Rikke H\u00f8gsted beskriver at fagpersoner, som arbejder i mentale h\u00f8jrisikojobs er i fare for at overinvolvere eller underinvolvere sig, n\u00e5r de sv\u00e6re f\u00f8lelser i arbejdet bliver for voldsomme. En fagperson, som hverken er over- eller underinvolveret, er tilpas involveret. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">En position, hvori fagpersonen er afstemt og i kontakt. If\u00f8lge Rikke H\u00f8gsted er det at v\u00e6re afstemt og i kontakt at:<br \/>\n<\/span><\/p>\n<div class=\"farve-boks\" style=\"background-color: #b8e1c0; padding: 16px 32px; border-radius: 24px; color: black; margin-bottom: 32px;\">\n<p><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><em>&#8220;man ved hj\u00e6lp af sin indlevelse er i stand til at lade sig ber\u00f8re f\u00f8lelsesm\u00e6ssigt i tillid til at man p\u00e5 baggrund af sin faglighed, vurderingsevne og ressourcer vil kunne p\u00e5virke situationen i den rigtige retning og en foruds\u00e6tning for at kunne udf\u00f8re et professionelt stykke arbejde og samtidig g\u00f8re det p\u00e5 en b\u00e6redygtig m\u00e5de&#8221;<\/em> (4).<\/span><\/p>\n<\/div>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">At v\u00e6re afstemt og i kontakt kan sidestilles med Hartmunt Rosas tanker om resonans, hvorfor det er meningsfyldt at over- og underinvolvering derved s\u00e6ttes i forhold til Rosas fremmedg\u00f8relse, som \u00f8ges ved acceleration (8). <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">At v\u00e6re afstemt og i kontakt for at kunne resonere, fordrer tid. Tid til at koble fornuft og f\u00f8lelser. Denne evne kaldes mentalisering og er et nyere begreb inden for sygepleje. Det har dog l\u00e6nge v\u00e6ret kendt i psykologi og p\u00e6dagogik. At kunne mentalisere er en almenmenneskelig evne, som de fleste behersker over tid, og kan forst\u00e5s som et udvidet empatibegreb.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">N\u00e5r man mentaliserer, fors\u00f8ger man at forst\u00e5 og fortolke egne og andres mentale tilstande. Det indeb\u00e6rer at kunne se sig selv udefra og den anden indefra, og spiller en central rolle i relationer og sociale interaktion, da det g\u00f8r det muligt at reagere hensigtsm\u00e6ssigt p\u00e5 egne og andres f\u00f8lelsesm\u00e6ssige behov og handlinger(9). Mentalisering spiller en vigtig rolle i vores forst\u00e5else og regulering af f\u00f8lelser og bidrager til resiliens.<br \/>\n<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Bedre mentaliseringsevne er forbundet med st\u00f8rre modstandsdygtighed mod psykiske problemer i m\u00f8det med stressende livsh\u00e6ndelser (9). Mentalisering m\u00e5 deraf betegnes som en beskyttelse i arbejdet med h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Studier udf\u00f8rt blandt sundhedspersonale under COVID-19-pandemien viste, at sundshedspersonale med h\u00f8jere evne til at mentalisere havde lavere niveau af depression, angst og stress. Modsat var lav mentaliseringsevne forbundet med h\u00f8jere niveauer af f\u00f8rn\u00e6vnte mentale sundhedsproblemer (10, 11). Dette underst\u00f8ttes af Rikke H\u00f8gsted, hvor mentalisering er en del af at v\u00e6re afstemt og i kontakt (4).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">At kunne mentalisere er alts\u00e5 en vigtig del af sygeplejen, men ogs\u00e5 en vigtig beskyttelse for sygeplejersken i forhold til h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger i arbejdet. Hartmut Rosa taler om at have et lydh\u00f8rt hjerte (12), hvorved fornuft og f\u00f8lelser kobles.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Mentalisering kr\u00e6ver pauser<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Evnen til at mentalisere er \u2013 meget forsimplet &#8211; i hjernes pr\u00e6frontal cortex med forbindelse til det limbiske system. S\u00e5fremt vores f\u00f8lelser er afstemte, og vi er trygge, har vi blodforsyning til at<\/span> <span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">opretholde vores mentalisering, hvilket bl.a. betyder, at vi kan koble fornuft og f\u00f8lelser. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Men n\u00e5r vi f\u00f8lelsesm\u00e6ssigt belastes og utryghed opst\u00e5r, vil blodforsyningen til mentalisering reduceres i tilf\u00e6lde af, at det autonome system skal aktiveres (13). Herved opst\u00e5r non-mentalisering, der kendetegnes som primitiv, rigid og uempatisk t\u00e6nkning. Non-mentalisering sker for os alle (9). <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Vi kender det fra udtryk som fx at tabe hovedet, miste m\u00e6let, v\u00e6re iskold eller i f\u00f8lelsernes vold. Ved non-mentalisering \u00f8ges misforst\u00e5elser og konfliktniveau samt risiko for udbr\u00e6ndthed og\/eller forr\u00e5else.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Dette underst\u00f8ttes af Rikke H\u00f8gsteds tanker om overinvolvering og underinvolvering. S\u00e5fremt sygeplejersken er non-mentaliserende, og dermed ikke afstemt og i kontakt, forringes kvaliteten af sygeplejen og sygeplejerskens evne til at h\u00e5ndtere h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">At arbejde med h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger \u00f8ger alts\u00e5 risikoen for non-mentalisering. Derfor er pauser s\u00e6rligt n\u00f8dvendige at holde, s\u00e5ledes at mentalisering kan opretholdes eller genetableres. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Blodforsyning til pr\u00e6frontal cortex med forbindelse til det limbiske system skal tilbage. Dette sker i trygge omgivelser og via mentaliserende andre. Her er pauser med kollegaer, som man f\u00f8ler sig forst\u00e5et og anerkendt af, vigtige. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Set ud fra mentaliseringsperspektivet har rygepauserne sat ramme om rettidig mulighed for at koble vores fornuft og f\u00f8lelser. I pauserne fik vi &#8211; via vores rygende kollegaer &#8211; genetableret og udviklet vores mentalisering, som gjorde os fagligt dygtigere og mere resiliens til at arbejde i de h\u00f8je f\u00f8lelsesm\u00e6ssige krav og belastninger, som sygepleje indeb\u00e6rer.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Hartmut Rosa vil sige at vi fik skabt resonans og Rikke H\u00f8gsted vil mene at vi blev afstemte og kom i kontakt.
