Delstudie 1 bestod af en sp\u00f8rgeskemaunders\u00f8gelse blandt 196 sygeplejersker ansat p\u00e5 intensiv afdelinger, hvor sp\u00f8rgeskemaet Self-Compassion Scale \u2013 Short Form (SCS\u2011SF) blev anvendt (4). Line\u00e6re regressionsanalyser blev gennemf\u00f8rt for at unders\u00f8ge sammenh\u00e6nge mellem self-compassion og demografiske samt arbejdsm\u00e6ssige faktorer. Compassion fatigue reflects a depletion of emotional resources following repeated exposure to suffering (3). Self-compassion, defined as a combination of self-kindness, common humanity, and mindfulness (4), has been highlighted in several studies as a central psychological resource that can enhance well-being and protect healthcare professionals from emotional exhaustion (5,6).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Aim<br \/>\n<\/strong><\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">To examine levels of self\u2011compassion among ICU nurses in Denmark and to explore how they experience self\u2011compassion and compassion fatigue in clinical practice.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Methods<br \/>\n<\/strong>A mixed\u2011methods design was used. Study 1 consisted of a survey among 196 nurses using the Self\u2011Compassion Scale \u2013 Short Form (SCS\u2011SF) (4). Linear regression analyses were performed to examine associations between self-compassion and demographic\/occupational factors. Study 2 involved seven semi\u2011structured interviews analysed using Braun &amp; Clarke\u2019s thematic analysis (7).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Results<\/strong><br \/>\n<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Substudy 1 showed that participants generally exhibited moderate to high levels of self-compassion and that frequent administrative workdays were significantly associated with higher self-compassion scores, with no statistically significant differences by age, gender, or experience. Substudy 2 resulted in five themes: Being in balance to provide nursing care; Meeting one\u2019s professional obligation; Space, structure, and time; The feeling of giving without receiving; and Honesty and realism. The themes illustrate how self-compassion is shaped by the interplay among individual resources, collegial relationships, and organisational conditions, and how compassion fatigue arises at the intersection of ethical demands, time pressure, and emotional exposure.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Conclusion<\/strong><br \/>\n<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The study indicates that self-compassion is a significant resource for well-being and professional sustainability among intensive care nurses. The combination of quantitative and qualitative data contributes to an integrated understanding of levels, associations, and mechanisms experienced, whereby self-compassion emerges as a phenomenon shaped by the interplay between individual and organisational factors. Compassion og self-compassion (SC) er t\u00e6t forbundne, idet SC underst\u00f8tter evnen til at udvise compassion over for andre uden at nedslide sig selv (5). SC best\u00e5r af tre kerneelementer: selvvenlighed, f\u00e6lles menneskelighed og mindfulness (6), og forskning viser, at SC kan reducere stress, udbr\u00e6ndthed og compassion fatigue hos sundhedsprofessionelle (7,8). Under COVID\u201119-pandemien fandt studier blandt intensivsygeplejersker, at h\u00f8jere SC var forbundet med bedre mental sundhed, h\u00f8jere engagement og forbedret jobpr\u00e6station (9).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Der er desuden tegn p\u00e5, at organisatoriske rammer og psykosociale kulturer &#8211; herunder kollegial og ledelsesforankret st\u00f8tte, tid til refleksion og muligheder for emotionel bearbejdning &#8211; spiller en afg\u00f8rende rolle i forhold til b\u00e5de compassion fatigue og compassiontilfredsstillelse (10,11). N\u00e5r sygeplejersker har adgang til st\u00f8tte fra kolleger og mulighed for debriefing, reduceres risikoen for belastningsreaktioner markant (12). Integration af de to datatyper finder sted i analyse og diskussion, hvor resultaterne sammenholdes med henblik p\u00e5 en samlet fortolkning.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Metode &#8211; delstudie 1 \u2013 kvantitativt studie<br \/>\n<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Design<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Delstudie 1 blev gennemf\u00f8rt som et tv\u00e6rsnitsbaseret sp\u00f8rgeskemastudie med det form\u00e5l at unders\u00f8ge graden af SC blandt sygeplejersker p\u00e5 intensiv afdelinger i Danmark samt identificere potentielle sammenh\u00e6nge mellem SC og sygeplejerskers demografiske og arbejdsm\u00e6ssige karakteristika. Et tv\u00e6rsnitsdesign er velegnet til at give et \u00f8jebliksbillede af forekomsten af psykologiske og organisatoriske faktorer i en given population (15).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Deltagere og rekruttering<br \/>\n<\/span><\/strong><span style=\"font-size: 16px;\">Rekrutteringen fandt sted via arbejdsmail gennem afdelingssygeplejersker p\u00e5 danske intensiv afdelinger i juni 2024. En rykker blev udsendt efter 20 dage. I alt gennemf\u00f8rte 196 sygeplejersker sp\u00f8rgeskemaet. Flertallet var kvinder, hvilket afspejler den generelle k\u00f8nsfordeling i sygeplejeprofessionen. Deltagerne repr\u00e6senterede et bredt spektrum af erfaring, uddannelsesniveauer og funktioner, herunder varierende omfang af administrative dage.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Instrument: Self\u2011Compassion Scale \u2013 Short Form (SCS\u2011SF)<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">SC blev m\u00e5lt ved brug af Self\u2011Compassion Scale \u2013 Short Form (SCS\u2011SF), et internationalt valideret instrument med 12 punkter, udviklet som en kortere version af den oprindelige Self-Compassion Score (SCS) (4). SCS\u2011SF-skemaet m\u00e5ler de seks underdimensioner selvvenlighed, selvkritik, f\u00e6lles menneskelighed, isolation, mindfulness og overidentifikation. Svarene gives p\u00e5 en 5\u2011punkt Likert\u2011skala, og der beregnes en samlet SC\u2011score.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Da en officiel dansk validering af SCS\u2011SF ikke<\/span> <span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">eksisterede under unders\u00f8gelsens tilblivelse i 2024, blev skalaen oversat efter principperne om forward\u2013back translation, som anbefalet i international metodologisk litteratur (16) og efter tilladelse (4). Valget af kovariater er baseret p\u00e5 tidligere forskning, der p\u00e5viser betydningen af demografiske og organisatoriske faktorer for compassion, trivsel og udbr\u00e6ndthed blandt sygeplejersker (2,5,10).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">For at unders\u00f8ge sammenh\u00e6ngen mellem SC og baggrundsvariabler blev der udf\u00f8rt en line\u00e6r regressionsanalyse (OLS), hvor SC\u2011score var den kontinuerte afh\u00e6ngige variabel. Valget af kovariater var a priori baseret p\u00e5 konceptuelle overvejelser om mulige confoundere (15). Sammenh\u00e6ng mellem kategoriske variabler og SC blev unders\u00f8gt med uafh\u00e6ngige t\u2011tests og ANOVA, j\u00e6vnf\u00f8r epidemiologiske standardmetoder (15).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I h\u00e5ndteringen af confounding og modelkontrol blev alle relevante kovariater inkluderet simultant, multikollinearitet blev kontrolleret via variansinflationsfaktorer og outliers blev identificeret visuelt, hvor ingen havde v\u00e6sentlig indflydelse p\u00e5 resultaterne. Til sidst fandtes, at residualplots og tests for homoskedasticitet ikke indikerede alvorlige brud p\u00e5 OLS\u2011antagelserne.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Analyse<br \/>\n<\/span><\/strong><span style=\"font-size: 16px;\">Den statistiske analyse blev udf\u00f8rt i Stata 16.1. Der blev gennemf\u00f8rt deskriptive analyser med beregning af middelv\u00e6rdier, standardafvigelser og procentfordelinger for SC-score og baggrundsvariabler. Derudover blev der anvendt bivariate tests samt multivariat regressionsanalyse. Stikpr\u00f8vegrundlag og fordelinger pr\u00e6senteres med angivelse af procentandele, og resultater rapporteres med estimater og konfidensintervaller, hvor relevant.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Etiske forhold<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Det kvantitative delstudie 1 var anonymt, frivilligt og uden biologisk intervention og var derfor ikke anmeldelsespligtigt til Videnskabsetisk Komit\u00e9. Databehandlingen fulgte g\u00e6ldende GDPR\u2011regler, institutionelle retningslinjer og lokale procedurer for databeskyttelse.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Deltagerne blev informeret om unders\u00f8gelsens form\u00e5l, anonymitet, frivillighed og h\u00e5ndtering af data. Da unders\u00f8gelsen omhandlede psykologiske forhold, blev deltagere med lave SC-scorer tilbudt psykologst\u00f8tte som en forebyggende foranstaltning, uden registrering af individuelle data, i tr\u00e5d med anbefalinger om etisk ansvarlig forskningspraksis (11).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Det kvalitative delstudie 2 adskiller sig etisk ved at indeb\u00e6re direkte kontakt med deltagerne og dermed st\u00f8rre fokus p\u00e5 relationelle og emotionelle aspekter. Studiet var ligeledes ikke anmeldelsespligtigt til Videnskabsetisk Komit\u00e9, da det ikke omfattede intervention eller biologisk materiale, og databehandlingen fulgte de samme GDPR\u2011krav og institutionelle retningslinjer. Der blev indhentet skriftligt informeret samtykke fra alle deltagere, som blev informeret om frivillighed, anonymitet, mulighed for tilbagetr\u00e6kning samt h\u00e5ndtering og destruktion af data. P\u00e5 grund af interviewenes potentielt f\u00f8lelsesm\u00e6ssige karakter blev det tydeliggjort, at deltagerne kunne afbryde interviewet n\u00e5r som helst, og der blev taget h\u00f8jde for eventuelt behov for st\u00f8tte i forbindelse med deltagelsen.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Metode &#8211; delstudie 2 \u2013 kvalitativt studie<br \/>\n<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Design<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Delstudie 2 blev udformet som en kvalitativ interviewunders\u00f8gelse med det form\u00e5l at unders\u00f8ge, hvordan sygeplejersker ansat p\u00e5 intensiv afdelinger oplever SC og compassion fatigue i deres daglige praksis. Kvalitative metoder er velegnede til at indfange komplekse, f\u00f8lelsesm\u00e6ssige og erfaringsbaserede dimensioner af sygepleje, som ikke kan m\u00e5les kvantitativt (18). Et semistruktureret design blev valgt for at skabe b\u00e5de retning og fleksibilitet i samtalerne.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Rekruttering og sampling<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I sp\u00f8rgeskemaet fra delstudie 1 kunne deltagerne angive, om de \u00f8nskede at deltage i et interview. 35 sygeplejersker meldte sig interesserede. P\u00e5 baggrund af stratificeret sampling blev 11 sygeplejersker udvalgt med henblik p\u00e5 maksimal variationsbredde, som anbefalet i kvalitativ metode (18). Syv sygeplejersker deltog i interviewene.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Dataindsamling<br \/>\n<\/span><\/strong><span style=\"font-size: 16px;\">Interviewene blev gennemf\u00f8rt online via Microsoft Teams og varede 45\u201370 minutter. De blev optaget og efterf\u00f8lgende transskriberet ordret. Der blev anvendt en semistruktureret interviewguide udarbejdet p\u00e5 baggrund af relevant litteratur, delstudie 1\u2011fund og forskningssp\u00f8rgsm\u00e5l (4,6,7,11). Tematiseringen blev dokumenteret detaljeret, se <\/span><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Bilag 2<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">. Analysen blev udf\u00f8rt manuelt af forskeren:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">1. Kodning af meningsfulde enheder<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Kodningen blev udf\u00f8rt linje\u2011for\u2011linje og fulgte en induktiv tilgang, hvor koderne udsprang direkte af data uden brug af en forh\u00e5ndsdefineret kodningsramme. Koderne omfattede bl.a. oplevelser af f\u00f8lelsesm\u00e6ssig belastning, faglig forpligtelse, organisatoriske rammer, kollegial st\u00f8tte og privatlivets betydning.<br \/>\n<\/span><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">3. Tematisk organisering af koder<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Koder blev organiseret i meningsklynger. Forel\u00f8bige temaer opstod omkring balance, forpligtelse, organisatoriske rammer, emotionel gensidighed og kulturel itales\u00e6ttelse.<br \/>\n<\/span><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">4. Den faglige og erfaringsm\u00e6ssige position giver en dyb indsigt i feltet, men indeb\u00e6rer ogs\u00e5 en risiko for forforst\u00e5elsesbias.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Refleksivitet blev h\u00e5ndteret gennem bevidsthed om egen forforst\u00e5else, som blev nedskrevet f\u00f8r analysearbejdet, og gennem en systematisk tekstn\u00e6r kodning, der skulle sikre, at temaer udsprang af data. Denne proces blev underst\u00f8ttet af gentagne genneml\u00e6sninger for at udfordre egne antagelser, kritisk refleksion over rolle, privilegier og faglige normer samt en tydelig skelnen mellem empiri og fortolkning, som fremg\u00e5r af dokumentationen af analyseprocessen. Refleksiviteten fungerede dermed som et centralt kvalitets- og transparensselement i analysen.<\/span><\/p>\n<p><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Dataintegration<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Integration af de to datas\u00e6t blev foretaget ved at sammenholde kvantitative m\u00f8nstre med kvalitative temaer. Denne analytiske kobling havde til form\u00e5l at belyse, hvordan statistiske sammenh\u00e6nge kan forst\u00e5s i lyset af sygeplejerskernes oplevede praksis.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Resultater delstudie 1 \u2013 kvantitativt studie<br \/>\n<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Deltagerkarakteristika<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I alt deltog 196 intensivsygeplejersker i sp\u00f8rgeskemaunders\u00f8gelsen. Flertallet var kvinder (92%), hvilket afspejler k\u00f8nsfordelingen i sygeplejerskefaget generelt. Deltagerne varierede i alder, erfaring og uddannelsesniveau: 62 % havde mere end 10 \u00e5rs erfaring, 56 % havde specialuddannelse i intensiv\u2011 eller an\u00e6stesisygepleje, og resten fordelte sig p\u00e5 grunduddannelse, diplomuddannelse, master,<\/span> <span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">kandidat eller ph.d.-niveau. text-strikethrough-none\">Selvom den multivariate analyse ikke p\u00e5viste statistisk signifikante sammenh\u00e6nge mellem SC og kovariater som alder, erfaring og uddannelsesniveau, peger de supplerende analyser p\u00e5 enkelte m\u00f8nstre, der kan have klinisk relevans. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Der ses eksempelvis en svag tendens til stigende SC med alderen, hvilket er i overensstemmelse med internationale studier (5). Det kan indikere, at livs- og arbejdserfaring over tid bidrager til udvikling af h\u00f8jere SC, selvom sammenh\u00e6ngen ikke kan p\u00e5vises statistisk i dette studie. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">En analyse af de deltagende sygeplejersker med lav SC (n = 12) viste, at de typisk havde 6\u20138 \u00e5rs erfaring, ingen havde uddannelsesniveau over special- eller diplomuddannelse og 75 % rapporterede om ingen administrative dage i deres arbejde.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Ligeledes peger de deskriptive data p\u00e5, at sygeplejersker med lav SC typisk har et moderat erfaringsniveau (6\u20138 \u00e5r), begr\u00e6nset eksponering for administrative dage og ingen videreuddannelse ud over special- eller diplomniveau, se<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong> Figur 5<\/strong>. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Det kan indikere, at b\u00e5de erfaringens karakter og uddannelsesniveau spiller en rolle i udviklingen af SC. S\u00e6rligt kan videreuddannelse t\u00e6nkes at styrke refleksionskompetencer, klinisk overblik og mestringsstrategier, som er centrale elementer i SC. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Endvidere viser fordelingen i <\/span><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Figur 5<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">, at sygeplejersker med h\u00f8jere uddannelsesniveau i h\u00f8jere grad har afsat tid til administrative dage, hvilket kan pege p\u00e5 en indirekte sammenh\u00e6ng mellem uddannelse, organisatorisk rolle og SC. Dog peges der p\u00e5, at erfaring og uddannelsesniveau kan have en betydning for SC, men at denne betydning er kompleks og muligvis indirekte medieret gennem faktorer som arbejdsfunktion, ansvarsniveau og eksponering for kliniske belastninger. Fremtidige studier med st\u00f8rre sample size eller longitudinelt design vil v\u00e6re relevante for at unders\u00f8ge disse sammenh\u00e6nge n\u00e6rmere. Resultaterne underst\u00f8tter desuden behovet for at forst\u00e5 SC i en organisatorisk kontekst (10,11).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Resultater &#8211; delstudie 2 \u2013 kvalitativt studie<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Den tematiske analyse resulterede i fem overordnede temaer, som tilsammen belyser, hvordan sygeplejersker p\u00e5 intensiv afdelinger oplever SC og compassion fatigue i deres kliniske hverdag. De fem temaer pr\u00e6senteres i det f\u00f8lgende.<br \/>\n<\/span><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Tema 1: At v\u00e6re i balance for at kunne yde sygepleje<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Sygeplejerskerne beskriver f\u00f8lelsesm\u00e6ssig og mental balance som en grundl\u00e6ggende foruds\u00e6tning for at kunne yde sygepleje af h\u00f8j kvalitet. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">N\u00e5r de selv har overskud, beskriver de, at de kan m\u00f8de patienter og p\u00e5r\u00f8rende med n\u00e6rv\u00e6r og ro \u2013 i tr\u00e5d med teorien om SC som en psykologisk buffer (6,7). Fundene peger dermed p\u00e5, at indsatser, der udelukkende retter sig mod individet, kan v\u00e6re utilstr\u00e6kkelige, og at organisatoriske forhold spiller en central rolle for b\u00e6redygtighed i praksis.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I forl\u00e6ngelse heraf viser studiet, at SC ikke kan forst\u00e5s uafh\u00e6ngigt af de vilk\u00e5r, sygeplejersker p\u00e5 Intensiv arbejder under. Compassion fatigue kan opst\u00e5, n\u00e5r et h\u00f8jt etisk og emotionelt ansvar udfoldes i organisatoriske rammer, der begr\u00e6nser muligheden for at yde fagligt og etisk forsvarlig sygepleje (3,13,19).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Begrebsm\u00e6ssigt er feltet fortsat pr\u00e6get af uklarhed. Noor et al. (2025) peger i et scoping review, der omfatter 43 studier, p\u00e5 et fortsat behov for begrebslig afklaring. Empati kan forst\u00e5s som evnen til at dele andres f\u00f8lelsesm\u00e6ssige tilstand, men kan ved vedvarende eksponering f\u00f8re til empatisk belastning, mens compassion repr\u00e6senterer en mere reguleret, prosocial respons, hvor intentionen er at lindre lidelse uden selv at blive overv\u00e6ldet (21). <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Hofmeyer et al. (2020) introducerer begrebet \u2018empathetic distress fatigue\u2019 som en mere pr\u00e6cis betegnelse for den belastning, sundhedsprofessionelle oplever.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Belastningen opst\u00e5r s\u00e6rligt ved overidentifikation med patienters lidelse samt utilstr\u00e6kkelig emotionel regulering. Centralt understreges det, at l\u00f8sningen ikke er at reducere graden af compassion, men derimod at styrke kompetencer inden for compassion, SC og emotionel regulering. Dermed peges der p\u00e5 et begrebsskifte, hvor compassion i h\u00f8jere grad forst\u00e5s som en del af l\u00f8sningen frem for problemet i omsorgsarbejde (22).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Metodiske begr\u00e6nsninger<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Det er centralt i kvalitetsvurderingen af mixed\u2011methods studier at tydeligg\u00f8re mulige metodiske begr\u00e6nsninger, som kan have betydning for fundenes styrke, overf\u00f8rbarhed og fortolkning (14,18). Frivillig deltagelse er en almindelig udfordring i tv\u00e6rsnitsstudier og kan f\u00f8re til en mindre repr\u00e6sentativ stikpr\u00f8ve (15).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Anvendt scoreredskab<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I den foreliggende unders\u00f8gelse er SCS-SF anvendt i en dansk overs\u00e6ttelse foretaget i 2024, idet der p\u00e5 dav\u00e6rende tidspunkt ikke forel\u00e5 en valideret dansk version. Efter dataindsamlingen er der imidlertid publiceret et studie (23), som validerer den fulde SCS-26 i en dansk kontekst. Dette peger p\u00e5 en metodisk styrke ved brugen af den l\u00e6ngere version, idet den udviser bedre psykometriske egenskaber og mulighed for mere nuancerede analyser. Omvendt er SCS-SF mindre ressourcekr\u00e6vende og mere anvendelig i praksisn\u00e6re kontekster. Det har ikke v\u00e6ret muligt at gentage dataindsamlingen p\u00e5 baggrund af den nye validering, hvilket udg\u00f8r en metodisk begr\u00e6nsning, men ogs\u00e5 afspejler forskningens tidsm\u00e6ssige vilk\u00e5r.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Selvrapporterede data<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">SCS-SF er et selvrapporteret instrument, som afh\u00e6nger af deltagernes egne vurderinger, hvilket kan indeb\u00e6re metodiske begr\u00e6nsninger forbundet med selvrapportering (16). <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Selvom SCS\u2011SF har god reliabilitet (4), kan selvrapporterede data ikke tolkes som objektive m\u00e5l for trivsel.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Tv\u00e6rsnitsdesignets begr\u00e6nsninger<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Tv\u00e6rsnitsdesign tillader ikke at unders\u00f8ge \u00e5rsagssammenh\u00e6nge, da m\u00e5ling af eksponering (fx administrative dage) og udfald (SC) sker samtidig (15). Dermed kan studiet ikke afg\u00f8re, om administrative dage \u00f8ger SC, eller om sygeplejersker med h\u00f8j SC i h\u00f8jere grad ops\u00f8ger administrative funktioner. Fundene peger p\u00e5 sammenh\u00e6nge, men ikke p\u00e5 retning.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Begr\u00e6nset interviewdeltagelse<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">11 ud af 35 respondenter blev udvalgt til at blive interviewet. Fire meldte afbud og syv sygeplejersker deltog i de kvalitative interviews. Selvom der blev anvendt stratificeret udv\u00e6lgelse med henblik p\u00e5 at sikre variationsbredde (18), rejser antallet af deltagere sp\u00f8rgsm\u00e5l om, hvorvidt der er opn\u00e5et fuld datam\u00e6tning. Det kan s\u00e5ledes ikke udelukkes, at yderligere interviews kunne have frembragt nye perspektiver eller nuancer, s\u00e6rligt fra de sygeplejersker, der af forskellige \u00e5rsager ikke havde mulighed for at deltage.<\/span> <span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Dataindsamlingens placering i en ferieperiode kan have begr\u00e6nset deltagelsen og dermed variationen i datamaterialet. Resultaterne b\u00f8r derfor fortolkes med opm\u00e6rksomhed p\u00e5, at enkelte erfaringer og synspunkter potentielt ikke er repr\u00e6senteret.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Forskerens forforst\u00e5else<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Forfatterens st\u00e6rke faglige tilknytning til intensivsygepleje, medlemskab af Sygeplejeetisk R\u00e5d og ekspertise inden for compassion (19) kan indeb\u00e6re risiko for bias i fortolkningen af data. Denne risiko blev fors\u00f8gt h\u00e5ndteret via systematisk refleksivitet, tekstn\u00e6r kodning og transparens omkring forskerpositionen, men kan aldrig elimineres fuldst\u00e6ndigt, hvilket er en generel pr\u00e6mis i kvalitativ forskning (18). Selvom unders\u00f8gelsen er gennemf\u00f8rt i en anden region end den, hvor forfatteren har klinisk erfaring, kunne dataindsamlingen med fordel have v\u00e6ret udf\u00f8rt af en person uden specialiseret viden om compassion eller sygepleje for at minimere risikoen for bias i dataindsamlingen.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Kontekstuel begr\u00e6nsning<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Studiet omhandler sygeplejersker ansat p\u00e5 intensiv afdelinger i en dansk kontekst og kan derfor ikke uden videre generaliseres til andre sygeplejespecialer, andre lande, andre typer sundhedsprofessionelle eller andre sektorer som kommune eller psykiatri. Alligevel kan fundene have relevans i lignende h\u00f8jintense kliniske milj\u00f8er.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Kombinationen af metoder<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Mixed\u2011methods design giver st\u00e6rk triangulering, men forskelle i datatyper (selvrapporteret skala vs. interviewdata) kan udfordre integrationen.
