making multidisciplinary teamwork essential. However, a multidisciplinary perspective is needed to manage the growing number of cases of patients with complex health care challenges (1).<\/span> To deliver the highest quality of health care across complex settings, there is a demand for health care to incorporate multidisciplinary approaches.<span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> These are defined as health workers from multiple professional backgrounds who work together with patients and families to explore multiple conditions and symptoms, and to define patients\u2019 preferences for treatment and solutions in a person-centred approach (2).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Multifactorial symptoms and multidisciplinary consultations<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Given the growing need for multidisciplinary approaches in complex healthcare, patients with multifactorial conditions highlight the importance of effective collaboration across specialities. This is particularly evident in the care of pelvic floor disorders (PFDs). Patients with PFDs can suffer from a range of multifactorial symptoms, e.g. urinary problems ranging from urinary incontinence to urinary retention, bowel problems ranging from faecal incontinence to obstipation, and pain from various compartments of the pelvic floor and subsequently affect their sexual health due to the various symptoms (3). The multiple problems significantly affect quality of life (4-6).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The patients do not necessarily have multiple conditions, but their symptoms can arise from multiple compartments of the pelvic floor, creating uncertainty about the best treatment options (3). A symptom, such as pelvic pain, which often leads to sexual problems, can originate from not only different anatomical and physiological challenges but also behavioural and psychological aspects. Thus, helping patients with PFDs is complex and involves a range of specialities, including gynaecology, urology, proctology, and sexology. Consequently, the various professional disciplines and their collaboration play an important role in the care of patients with PFDs.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">We therefore argue that the field of PFDs has needs comparable to those observed in multi-disciplinary treatment and care across complex health care settings. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Given these considerations, this article focuses on multidisciplinary consultations, which must explore patients\u2019 multiple complex symptoms and preferences to offer the best options. Such consultations require effective working relationships across professions and specialities. However, the concepts used to describe such interprofessional working relationships \u2013 including collaboration, coordination and cooperation \u2013 have been applied inconsistently in the literature (7). In this paper, we adopt the term \u201ccollaboration\u201d as defined by Casta\u00f1er et al.:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">\u201ccollaboration refers to voluntarily helping others to attain a common interorganisational goal or a private goal\u201d<\/span><\/em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> (p. 994).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">We consider this definition to reflect the requirements of multidisciplinary collaboration in health care, where professionals are expected to support one another and work towards shared goals to ensure optimal patient care. Collaboration depends on effective communication, shared situational awareness and alignment around common clinical goals (8).<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&nbsp;However, such collaboration may also give rise to intra- and interprofessional conflicts, leading to inefficiency and ultimately reduced quality of care (9).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">A review by Gregorieu et al. shows that physicians and nurses find it important to collaborate; however, they do not agree on their roles. The physicians tend to regard themselves as the principal decision makers, thereby perpetuating a hierarchical structure (10). That said, physicians have also been found to be the drivers in negotiating and dealing with overlaps in areas of responsibility. In contrast, nurses are shown to be more active in bridging gaps between professional perspectives and transferring knowledge between professionals (11).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The frames of collaboration in the context of PFDs have been studied previously. In a Danish pelvic floor clinic, the organisational setup, including a multidisciplinary consultation and a moderator, was explored through interviews with healthcare professionals. The set-up proved to support patient-centeredness and collaboration (12). It was found that the moderator identified and prioritised which professions\u2019 input was needed to meet the patient&#8217;s preferences and kept an overview of the dialogue and timelines. Furthermore, this study showed that using a moderator seemed to increase the focus on multidisciplinary collaboration. However, it was unexplored which supporting factors came into play during the consultation.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">A substantial body of research has examined teamwork, team collaboration, and performance (13). Still, new contexts and organisational set-ups continue to emerge, underscoring the need for ongoing investigation to ensure that research keeps pace with contemporary developments(14).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Healthcare remains largely organised around single-disease specialities, despite the growing need for multidisciplinary approaches in complex conditions such as pelvic floor disorders. However, the drivers of effective collaboration in such settings are not well understood.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">To our knowledge, there is a lack of evidence on how collaboration unfolds in practice within moderatorled consultations for patients with PFDs, and which specific organisational and re-lational factors drive this process. We therefore seek to understand what unfolds within this framework and which drivers may be mobilised to strengthen collaboration.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Thus, for this study, we developed the research questions:<br \/>\n<\/span><\/p>\n<ul>\n<li class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">How is collaboration experienced during joint multidisciplinary consultations?<\/span><\/li>\n<li class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">How does the moderator facilitate collaboration in the joint multidisciplinary consultations?<\/span><\/li>\n<\/ul>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Aim<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">This study aimed, through the perspectives of health care professionals participating in joint multidisciplinary consultations guided by a moderator, to identify drivers that may serve as specific contributors to multidisciplinary collaboration.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Methods<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The study is a qualitative interview study. The method is inspired by Braun and Clarke\u2019s original work on thematic analysis (15). It uses the operationalised hermeneutic approach by Steinar Kvale &amp; Svend Brinkmann (16) to analyse interviews with healthcare professionals within a multidisciplinary team (MDT), conducted with a moderator. The team conducts joint multidisciplinary consultations at a pelvic floor clinic (PFC). Interview data originated from a previous interview study (12) and were reanalysed in line with this study&#8217;s aim. Methodological framework and reporting were based on the COREQ checklist (17).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Setting<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The PFC was established in 2019 at a hospital in Denmark. The MDT conducts multidisciplinary consultations once a week. The PFC has an annual capacity of approximately 250 joint mode-rator-guided, multidisciplinary consultations, primarily with female patients presenting with a combination of urinary and bowel problems. Ten appointed healthcare professionals from different departments and specialities, with different professions, belong to the PFC and are involved in these multidisciplinary consultations.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Once a week, five of these healthcare professionals constitute the team that performs the multidisciplinary consultation: three consultants; one gynaecologist, one urologist, one proctologist, one specialist nurse, and one specialist physiotherapist. Due to initial problems with tracking time for each consultation, qualitative work was facilitated by the Hospital&#8217;s Qualitative Department and by PDSA circles (18). This process identified the need for a chairman, and after one year, the concept of a moderator was implemented.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">One team member moderates each multidisciplinary consultation; intentionally, the moderator is never one of the consultants, but either a nurse or a physiotherapist. To our knowledge, this concept has not been implemented in other PFC settings.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Data collection<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">To obtain a nuanced and valid picture of the collaboration in the study, eligible informants were all healthcare professionals (consultants, nurses, and physiotherapists) who were involved in most multidisciplinary consultations during the year before the interviews. Nine of the ten healthcare professionals from the PFC were eligible and had been involved in between 100 and 250 of the total 250 consultations. One consultant was excluded because she participated in only 18 joint consultations in the preceding year. All nine eligible healthcare professionals were invited and consented to participate. In the end, one eligible female consultant could not participate after all because of the workload. Thus, interviews were performed with three consultants, four nurses and one physiotherapist.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">A semi-structured interview guide was developed in collaboration with healthcare professionals who had prior experience conducting semi-structured interviews and working in the field of PFDs. The format was inspired by Tanggaard and Brinkmann (19), see <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Table 1.<\/strong> <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The semistructured format allowed for both guided and emergent questions, ensuring focus on the research domains while enabling participants to elaborate freely. Interpretive follow-up questions were used to validate understanding, and interviews concluded with open prompts to invite additional perspectives. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Thereby, the interview guide included introductory, follow-up, and descriptive questions as recommended by Tanggaard and Brinkmann (19). During the interviews, further probing questions were asked. Encouragement was provided through verbal affirmations and nodding from the interviewer to demonstrate active listening toward the informant. At the same time, attention was paid to ensure that the research questions were addressed.<\/span><\/p>\n<p><img data-recalc-dims=\"1\" decoding=\"async\" class=\"wp-image-24072 size-full aligncenter\" src=\"https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/Table-1.png?resize=703%2C436&#038;quality=80&#038;ssl=1\" alt=\"\" width=\"703\" height=\"436\" srcset=\"https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/Table-1.png?w=703&amp;quality=80&amp;ssl=1 703w, https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/Table-1.png?resize=300%2C186&amp;quality=80&amp;ssl=1 300w, https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/Table-1.png?resize=600%2C372&amp;quality=80&amp;ssl=1 600w\" sizes=\"(max-width: 703px) 100vw, 703px\" \/><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">According to the hermeneutic approach, interpretive questions were also employed when they arose during the interview to confirm the accurate understanding of the responses. Finally, each informant was asked whether there was anything they wished to add to the topic. This served both to open up perspectives that the interviewer had not anticipated and to allow the informants to correct or nuance their statements.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The co-author and nurse (BTE) collected data from the eight interviews. BTE was employed outside the PFC in a speciality other than PFDs and therefore did not know the informants, was na\u00efve to their work situation or the practice of joint consultations. Although BTE\u2019s clinical background as a nurse provided contextual insight into professional roles and challenges in the field, it may also have influenced the emphasis placed on topics during the interviews. To mitigate potential bias and power dynamics, the interviewer had neither current nor previous employment in the department under study or in any PFC.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">BTE<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> conducted semi-structured individual interviews, either face-to-face or by phone, in a secluded office. Interviews were audiorecorded, and recordings were anonymous to others from the PFC, including the team member who is also a co-author (MH). The importance of this confidentiality lies in ensuring free speech for all interviewees.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Data analysis<br \/>\n<\/span><\/strong><span style=\"font-size: 16px;\">A naive listening of the audio recordings (Phase 1) (Braun &amp; Clarke) was conducted, followed by transcription of the interviews. Transcription was performed by BTE. Phase 2 included several readings to identify recurring patterns and code important text sequences. In Phase 3, themes were identified using an interpretive approach seeking not only \u2018what is said\u2019 but also to understand \u2018what is the meaning\u2019 of the text, which is also called the latent level in the coded text. The hermeneutic spiral was applied to understand and interpret the meaning the interviewee ascribed to their experiences.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Through a process in which the nurse interviewer moved between the parts and the whole, each data unit was subjected to in-depth reflection to uncover its unique significance. Subsequently, these interpreted elements were synthesised into a new, coherent interpretative whole, providing a deeper understanding of the phenomenon (Phase 4).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The inductive approach was data-driven, and codes were organised into themes. BTE continuously tested codes and themes in a hermeneutic spiral throughout the analysis (Phase 4). To ensure the integrity of the analysis, the researcher (BTE) engaged in continuous reflexivity. BTE\u2019s own professional background as a nurse and pre-understandings of multiprofessional collaboration were critically acknowledged, as these preconceptions inevitably shape the interpretation of the interview data and the identification of emerging themes. BTE named the themes (Phase 5), which were reported (Phase 6) in accordance with the COREQ checklist. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Data extraction with coding examples is illustrated in <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Table 2<\/strong>. Analysis and theme definition were initially performed by BTE and followed by discussions between BTE and an outside researcher with a background in the social sciences, the co-author (BRT).<\/span><\/p>\n<p><img data-recalc-dims=\"1\" loading=\"lazy\" decoding=\"async\" class=\"wp-image-24073 size-full aligncenter\" src=\"https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/TABLE-2.png?resize=335%2C197&#038;quality=80&#038;ssl=1\" alt=\"\" width=\"335\" height=\"197\" srcset=\"https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/TABLE-2.png?w=335&amp;quality=80&amp;ssl=1 335w, https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2026\/04\/TABLE-2.png?resize=300%2C176&amp;quality=80&amp;ssl=1 300w\" sizes=\"(max-width: 335px) 100vw, 335px\" \/><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Ethics<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">According to Danish law, approval from the National Committee on Health Research Ethics was not required for this project, as no biomedical intervention was conducted. The study was performed in accordance with the Declaration of Helsinki (20).<\/span> <span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">All participants gave informed, written consent. To address the challenge of anonymity, given the small number of interviewees, an outsider interviewed, transcribed, and pseudoanonymized the data.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Afterwards, an analysis was performed by two outsiders. All participants were informed of the possibility of withdrawing consent and of withdrawing specific statements from the interview, or the entire interview, if they regretted anything afterwards. Data was handled in accordance with GDPR and Danish Law and stored on a secure regional server (SharePoint site). Data permission was obtained (Journal number: 19\/46391).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Findings \/ Analysis<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The qualitative data were collected in November 2023. The PFC has had joint MDT consultations since 2019. The eight healthcare professionals interviewed had been working together since the start-up of the implementation. They were interviewed individually, each interview lasting from 16 to 48 minutes.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The informants had individually worked with patients with PFDs for between 5 and 33 years. Four interviews were conducted face-to-face, and four were conducted by phone. Five informants were identified as potential moderators (four nurses and one physiotherapist), and three were team members who did not hold the moderator role, as they were consultants. No informants have withdrawn any consent for certain statements or for the entire interview.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Thematic analysis<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Based on the analysed data, we formed one main theme with five subthemes, see <\/span><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Figure 1<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">. The themes are analysed in sections, and findings for each section are supported by selected quotes. Working in a multidisciplinary team setting requires that one&#8217;s own professional identity is not the most important, and that everyone works towards common ground to meet the patient&#8217;s needs. Thus, if the highest priority is one\u2019s own professionalism, this will be an inhibiting factor for collaboration, which the quote below reveals:&nbsp;<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span style=\"color: #008080;\"><strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">I think it\u2019s important to forget about your job title; everyone is equal- physiotherapists, nurses, doctors, everyone is on the same level when collaborating with the patient, and each person contributes to the whole. It only works if everyone collaborates. And that\u2019s the most important thing. That\u2019s why I believe our pelvic floor clinic works so well\u2026 because we like each other and our collaboration works well<\/span><\/em><\/strong><\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> (Informant F &#8211; without the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Respect and good interpersonal relationships are required for satisfactory collaboration. To further strengthen collaboration, recognising each other as equals and entrusting the moderator role during consultations promotes unity and a shared goal. Having a moderator is valuable for the PFC because she can maintain an overview while ensuring the relevant perspectives are considered:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><span style=\"color: #008080;\"><em><strong>(\u2026) Our moderator also keeps an eye on the other colleagues. (\u2026) But I think the moderator more or less manages everything; I pay attention to my recommendations, and the moderator manages all recommendations from all colleagues \u2026<\/strong><\/em><\/span>(Informant F &#8211; without the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Developing an understanding of each other and gaining a broader overview enables collaboration and a focus on the patient\u2019s pathway. The clinical challenges and the necessary processes require openness to this process. This generates new organisational resources within a complex system that is inherently dependent on effective multiprofessional collaboration. However, an adjustment period is important for aligning roles and contributions in the collaboration:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><em><span style=\"color: #008080;\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">\u2026 and professionally, of course, since we all participate in the same consultations, we inevitably start to learn how things are connected when we see things from different perspectives, and that\u2019s what makes this quite exciting, right? <\/span><\/strong><\/span><\/em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">(Informant G &#8211; without the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\">This shared participation in clinical consultations catalyses a collective learning process, in which the convergence of diverse professional perspectives transforms individual observations into a holistic understanding of the patient\u2019s pathway, ultimately fostering an organisational culture that views complexity as an opportunity for multiprofessional growth.<\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The organisation of the PFC and the framework for team collaboration are valuable contributors to the unit\u2019s ability to work in a robust, structured manner. As a further enhancement to organising the PFC, a daily manager ensured that the practical aspects of the PFC were handled, e.g., vacations and short-term sick leave, creating space for a common focus on new developments and relationship-building. The staff had requested a manager to gain better control over the context that could influence the PFC and support them as a team, arranging staff meetings, etc:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span style=\"color: #008080;\"><em><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">.<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">..we wanted an operational manager. We said it can\u2019t work if we each have to go back to our own departments and figure out holiday schedules, because it needs to match the clinic, right? I think collaboration is really important here<\/span><\/strong><\/em><\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> (Informant A &#8211; with the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Subtheme 1 &#8211; Personal relations<br \/>\n<\/span><\/strong><span style=\"font-size: 16px;\">Interviews showed that knowing each other becomes important and influences dialogue, creating an understanding of each other\u2019s skills. These personal relationships across different professions and specialities seem to have become a resource that positively influences collaboration. Personal relations are not inherent within the team and must be nourished over time and in different ways:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\" style=\"color: #008080;\">[A good collaboration] \u2026is when we know each other and are aware of each other&#8217;s skills. There is a really strong acceptance of diversity because we all have very different competencies<\/span><\/em><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> (Informant E &#8211; with the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Analysis of the findings emphasises that adjustment and getting to know each other are crucial to developing recognition and mutual respect. For moderators, this adjustment period provided an opportunity to get to know colleagues better, particularly to learn to read team members&#8217; nonverbal signals during consultations, allowing the moderator to bring the right person into the professional discussion.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The moderator maintains an overview and ensures that all relevant perspectives are heard and included. While each professional can focus on problem-solving, the moderator seems to incorporate all suggestions into a more unified picture to present to the patient. Here, she uses her empathetic skills and personal relationships with team members:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\" style=\"color: #008080;\">\u2026 so I see a slight rise of an eyebrow because that&#8217;s when someone else [another team member] wants to say something, right? So I also try to say something there\u2026 because it\u2019s important in the conversation to pick up on the signals the others [team members] are sending [about each discussion with the patient], because otherwise, it doesn\u2019t work well together <\/span><\/em><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">(Informant A &#8211; with the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The analysis of &#8220;nonverbal signals&#8221; (such as a raised eyebrow) demonstrates that close relationships enable implicit coordination. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">As team members build personal relationships, a foundational trust is established, making it safe to bring specific professional expertise into play. Knowledge of each other&#8217;s skills transforms &#8220;acceptance of diversity&#8221; from a polite norm into an active pursuit of each other&#8217;s perspectives. This creates a driver in which the individual feels pre-emptively recognised, removing the need to &#8220;defend&#8221; professional territory and opening the door to more fluid knowledge sharing.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Subtheme 2 &#8211; Professional Competencies<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Being a team member requires being well-grounded in one&#8217;s speciality and profession. However, it seems that, at the same time, it is imperative to set aside one&#8217;s speciality or profession to be open to the input of others. Equality means ensuring all professions and specialities are heard, not to promote individual fields, but to collaborate on the best possible solutions for the patient.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Understanding the other team members\u2019 competencies contributes to creating effective collaboration. Professional competencies seem to be enforced by reflecting closely together. Over an extended period, this provides insight into the multiple clinical specialities and into respect for one another. Building personal relationships enhances knowledge of specialities and acceptance despite the differences between team members:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span style=\"color: #008080;\"><em><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">It\u2019s about being equal in status and respecting each other and the patient\u2026 there is a time and place to listen to each other&#8217;s reasoning and opinions, and you can ask questions and obtain more information\u2026 about the background of the information the patient receives. It\u2019s an open, trusting relationship that has been built over time\u2026 <\/span><\/strong><\/em><\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">(Informant C &#8211; with the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The quote illustrates how developing long-term, trust-based relationships, grounded in respect for each other as persons, creates a safe space. This indicates that professional competencies are strengthened through close mutual reflection. This is a central driver, as collaboration here ceases to be merely the coordination of tasks and instead becomes a learning process. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">By gaining insight into each other&#8217;s reasoning over time, the team develops a shared &#8220;clinical language.&#8221; This enables the asking of qualified questions across specialities, which improves the basis for decision-making and ensures that the patient does not receive conflicting information.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Subtheme 3 &#8211; Respect<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">However, successful collaboration also depends on participants demonstrating mutual respect and trust in each other. The moderator ensures that all relevant perspectives are seen and that they contribute to the discussion. Even if a shared decision for planning examinations and treatments is difficult to reach, an agreement is essential. It must be reached before the end of the consultation in the patient\u2019s best interest:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span style=\"color: #008080;\"><strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Basically, the collaboration is founded on respect for each other&#8217;s areas of expertise and on understanding how your own perspective on a problem interacts with others&#8217;<\/span><\/em><\/strong><\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&nbsp;(Informant G &#8211; without the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\">This foundational respect for specialised expertise, coupled with an awareness of how individual perspectives intersect, creates the necessary professional alignment required to reach a unified clinical decision within the time-sensitive framework of the consultation. <span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Mutual respect is not just a social convention; it is a functional driver that reduces friction in the decision-making process. When team members trust each other\u2019s expertise, they move away from a &#8220;gatekeeping&#8221; mindset toward a more collaborative orientation.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">This respect allows for a &#8220;professional alignment&#8221; in which individual perspectives are not seen as competing but as intersecting layers. This creates a psychological infrastructure that enables agreement even when clinical opinions initially diverge.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Subtheme 4 &#8211; One common goal<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">It was important for collaboration that team members shared the same goal, working together to merge goals and options regarding every treatment option for their mutual patient. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The moderator brings together the healthcare professionals\u2019 perspectives, focusing on the patient&#8217;s overall goal, as the patient\u2019s goals and preferences guide and shape the consultation. However, it took time for the staff to adjust their expectations for collaboration in the consultation and to find their new roles within it:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\" style=\"color: #008080;\">\u2026 it\u2019s ultimately the patient\u2019s condition and treatment, not necessarily about finding the best solution as a urologist or surgeon, but more about finding the best solution for the patient\u2019s needs <\/span><\/em><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">(Informant F &#8211; without the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Subtheme 5 &#8211; Moderator grounded in caring competencies<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The role of the moderator emerged as central to effective team collaboration. Findings show that professions with caring competencies and empathic skills align well with the role of a moderator. The moderator can care for the patient, build trust in the relationship, provide information, and ensure the patient feels supported whilst maintaining an overview of the consultation&#8217;s progress to ensure that the patient\u2019s expectations are heard and, if possible, met. The moderator has to focus on the overall goal of gathering all relevant input during the consultation and ensure that a plan for the next steps is developed:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span style=\"color: #008080;\"><em><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Of course, that supportive or care-giving role that\u2019s inherent in nursing is still there. It\u2019s still the nurse [as moderator] who makes sure the patient is informed along the way, has someone holding their hand if needed, and is on board with everything happening. And then, when we start planning what comes next, it\u2019s the nurse again who makes sure that what we suggest aligns with the patient\u2019s expectations and their capabilities <\/span><\/strong><\/em><\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">(Informant G &#8211; without the moderator function).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">In the PFC, some of the moderator&#8217;s functions are compared by the interviewees to those of a project leader, who ensures project progress. The moderator can focus on the patients\u2019 needs while gathering information from different clinical specialities and still have an overview of the conversation\u2019s direction.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">This role is anchored in caring competencies, relational awareness, ethical attentiveness, and a commitment to shared well-being, which are strongly and fundamentally associated with nursing skills, but not exclusive to healthcare professionals with a nursing education. These subthemes describe the drivers of collaboration within the team and how the moderators, with their caring competencies and relational awareness, influence it.<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&nbsp;Collaboration is supported by knowing each other personally, understanding each other\u2019s competencies, and respecting and accepting them. Subsequently, the moderator supports team members in working together toward a common goal.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Discussion<br \/>\n<\/span><\/strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Drivers in collaboration<br \/>\n<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Openness and shared goals<br \/>\n<\/span><\/em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Based on the analysis, we see that openness to the process and to the colleagues and their competencies plays an important role. This can be seen as a driver in facilitating the possibility for collaboration. It furthermore provides a broader overview of the work required to manage treatment coherently and in a connected manner across specialities.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The American systems scientist Peter Senge argues that high organisational performance can be achieved when, amongst other conditions, we are aware that any minor actions or adjustments can affect the system as a whole, when staff have shared goals and values and when team learning that enables members to \u201cthink together\u201d is actively cultivated (21). This is in line with what we found in the multidisciplinary set-up. Here, the staff experienced shared goals and values. <\/span>Furthermore, this seemed to result in more flexible professional boundaries, likely emerging alongside increased openness and a broader, more holistic perspective on the entire patient treatment.<\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The Moderator as an Organisational Strategy<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">A new review by Wei et al. found that, to improve collaboration, effort is required from the organisation, the team, and everyone within the team (22). <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Based on our analysis, we propose that the moderator is a catalyst for both individuals and teams to succeed and, as such, represents an organisational strategy to foster high-performing collaboration. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The analysis indicates that personal relationships play a crucial role in both communication and decision-making during consultations. When functioning well, these relationships may be considered as one or more drivers for effective collaboration.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Relational Coordination<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">As highlighted by Professor of Management Jody Hoffer Gittell, relational coordination is fundamental in healthcare, facilitating the integration of the diverse competencies required for multidisciplinary treatment and care (23). <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Gittell distinguishes between multidisciplinary and interdisciplinary approaches. Interdisciplinary collaboration is characterised as a model in which professionals work independently, driven by shared goals and knowledge. In contrast, in a multidisciplinary setting, each profession operates within its own boundaries, contributing alongside or on top of the work of other healthcare professionals.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">In this article, we use the term &#8220;multidisciplinary&#8221; to encompass what Gittell describes as &#8220;interdisciplinary&#8221;. However, we chose to use the term multidisciplinary because it emphasises the involvement of multiple specialities and professional groups, reflecting the need to address multiple conditions and multiple symptom profiles within patient care.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Her research demonstrates that relational co-ordination, defined as shared goals, shared knowledge, and mutual respect, when combined with effective communication, is a key enabler of high-quality, efficient healthcare delivery. This is consistent with our findings, which also emphasise shared goals and mutual respect. During the implementation period, the staff have come to know one another and developed an understanding of the contributions each can make and the clinical areas in which they can contribute with their expertise. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">They have developed mutual respect for one another\u2019s competencies and, together with the moderator, established an approach that ensures effective communication.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Facilitating collaboration<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Our findings also show that the moderator has an important, independent role in facilitating collaboration and success in the consultation. Gittell and colleagues argue that the impact of interdisciplinary coordinators \u2013 in our context referred to as moderators &#8211; significantly increases relational coordination, thereby promoting interdisciplinary collaboration (23).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">According to Gittell, an interdisciplinary coordinating role involves gathering information about the patient, passing information on to other professional groups, and deciding or determining who should make which decisions. Furthermore, Gittel\u2019s research shows that the use of an interdisciplinary coordinator increased patient satisfaction. The thematic analysis and findings from this study supported the view that personal relationships positively influenced team collaboration, leading to the patient benefiting most from the multidisciplinary consultation.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Personal relationships and Trust<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">An Irish study by Gleeson and colleagues confirms our findings. Gleeson&#8217;s study showed that personal relationships between professional groups promoted interprofessional communication (24). When colleagues had a strong personal relationship, they generally communicated and worked more efficiently. Other organisational researchers argued that personal relationships and competencies in building mutual understanding fostered trust among team members, which played a key role in mediating knowledge transfer and sharing (25).<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&nbsp;Moreover, the more confident team members are in each other, the more willing they are to share knowledge (26).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Cultural Influence and Hierarchy<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">In the introduction, we referred to a study by Gregoriou and colleagues who reported a lack of agreement between nurses and physicians regarding their respective roles in collaborative practice (10). <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">However, our findings do not support this observation. Throughout the interviews, nurses and physicians expressed closely aligned expectations of one another and described their collaboration as mutually satisfactory. The study by Gregoriou and colleagues is a systematic review on interprofessional collaboration, searching for articles in English and Greek.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The study includes articles from diverse settings, such as Pakistan, Egypt, and the USA. Compared to these countries, Denmark is characterised by a low power-distance culture, indicating limited acceptance of hierarchical authority and unequal power. Furthermore, Denmark exhibits low uncertainty avoidance, meaning that Danes are tolerant of behaviours and opinions that differ from their own (27).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Therefore, we argue that it might be easier for professionals from different disciplines to collaborate in multidisciplinary consultations in Denmark than in countries with high power distance and strong uncertainty avoidance. This also indicates that the study&#8217;s context is pivotal and, consequently, constrains the extent to which the findings can be generalised or interpreted in other settings. Furthermore, Gittell stresses that one must consider important elements such as local culture, management structures, and the \u2018chemistry\u2019 of relationships, which will considerably influence collaboration across different settings and teams (23).<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">&nbsp;This might also be the case in our setting; therefore, we cannot dismiss the possibility that these factors influenced the collaboration.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Synthesis of Findings<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Summing up, the analysis showed that collaboration was experienced as open, respectful, and mutually supportive, with staff appreciating shared goals and understanding each other\u2019s competencies. The moderator facilitated communication, coordinated decision-making, and catalysed team- and individual performance. Key drivers of multidisciplinary collaboration included relational coordination, trust, shared knowledge, and mutual respect.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Together, these factors enabled flexible professional boundaries and a holistic approach to patient care. Here, the overarching theme, <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Re-sources in Collaboration<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">, represents the study\u2019s central analytical contribution by demonstrating how the interplay between relational coordination and organisational facilitation is the primary driver of effective multidisciplinary practice. Our findings suggest that when an open, non-hierarchical culture is coupled with a moderator, it fosters a dynamic environment. Within this framework, flexible professional boundaries and mutual trust enable team members to &#8216;think together,&#8217; transforming individual expertise into a coherent, holistic resource for the patient\u2019s pathway.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Forming collaboration<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Another driver for establishing good collaboration in the PFC is a well-implemented team. The interviewed team members experienced that an extended adjustment period was necessary before arriving at the current structure for the MDT consultations at the PFC. Things weren\u2019t just working from day one, even though members were competent and positive about the implementation. This aligns with Tuckman&#8217;s theory of team formation and functioning (28, 29). According to Tuckman, a team can go through several phases after its formation and must reach the fourth phase before it can produce effectively. The five phases are:<\/span><\/p>\n<ul>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Forming &#8211; identifying the task and boundaries for interpersonal behaviour.<\/span><\/li>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Storming \u2013 intergroup adjustments, conflict, individuality and resistance to group structure.<\/span><\/li>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Norming \u2013 group cohesion is developed, and the group becomes an entity with acceptance of group norms.<\/span><\/li>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Performing \u2013 the group is now problem-solving; members are flexible and can perform the tasks that are needed.<\/span><\/li>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Adjourning \u2013 separation, the team is dissolved.<\/span><\/li>\n<\/ul>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\">Groups may move back and forth between developmental stages, particularly when group composition changes or new members are introduced. Various factors, such as group composition, duration, and the nature of the task, can influence the pace and sequence of stage progression.<span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> Consequently, when new members join a group, the team may revert to earlier stages to reestablish norms, roles, and interpersonal dynamics.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">They found that nurses serve as a valuable resource and that nurses themselves perceive one of their key roles as facilitating the clinical work of other healthcare professionals by coordinating patient care (31). Furthermore, the nursing profession often involves multifaceted work, including physical, emotional, cognitive, and organisational tasks, or a combination of these (32). In this study, we found that it is essential for moderators to care for the patient, build trust, and maintain an overview of the consultation.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Drivers of multidisciplinary collaboration<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Moderators need to combine the emotional, cognitive and organisational work to handle the tasks embedded in the moderator role. Not exclude nurses but also other professions may<\/span> <span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">master the necessary competencies even though these competencies are not essential parts of their education. Effective moderation relied on empathic, patient-centred and coordinating competencies. The moderator facilitated collaboration by maintaining an overview, building trust, and integrating contributions across professions. Key drivers included relational skills, care orientation, and coordination capacity.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">To sum up, multidisciplinary collaboration was driven by a combination of relational, organisational, and individual factors that together enabled high-performing teamwork. Openness, shared goals, and mutual respect fostered relational coordination, trust, and flexible professional boundaries. The moderator functioned as a central organisational catalyst, facilitating communication, coordination, and decision-making across disciplines.<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> Team maturation over time, supported by stability and iterative structural adjustments, further strengthened collaborative capacity. Collectively, these interrelated drivers supported a holistic, integrated approach to patient care within the studied context. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Taken together, multidisciplinary collaboration was driven by the interplay of five key factors: professional competencies, mutual respect, personal relationships, a shared common goal, and the presence of a moderator.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">In the PFC, the team worked through different structures and set-ups before settling with a moderator and the function as such, and according to findings, they probably have reached the \u201cPerforming phase\u201d, adjusting to the context and what is needed to enhance the task completion. However, it has taken some effort to reach this point. According to Tuckman\u2019s theory, potential future changes in the task, interpersonal dynamics, or other structural conditions do not preclude the possibility that PFC consultations may be compromised. This could require the team to re-engage in developmental processes to reattain the performing stage. The team&#8217;s maturation enabled effective multi-disciplinary collaboration, leading to a high-performing phase. Key drivers included time, stability, and iterative structural adjustments. Changes in team composition may, however, require renewed developmental processes to sustain performance.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Moderator skills<br \/>\n<\/span><\/strong><em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Caring competencies<br \/>\n<\/span><\/em><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Caring competencies and empathic skills seem essential for effective moderation in joint consultations, emphasising values and interpersonal skills that extend beyond a single profession. The analysis indicated that the nursing profession is particularly well-suited to the moderator role. A Danish PhD thesis on nurses\u2019 role in interorganisational coordination reported that nurses engage in both formal and informal interorganisational coordination (30). <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The thesis demonstrated a commitment to practical solutions, maintaining oversight, and safeguarding appropriate care and treatment (30). The Danish Centre for Shared Decision Making explored, through an anthropological lens, how nurses\u2019 roles in involving patients in decision-making are shaped and expressed in practice (31).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Recognition of each other\u2019s competencies and the development of personal relationships fostered trust and effective communication, while mutual respect and a shared goal aligned efforts across disciplines. The moderator acted as a central coordinator, enabling the integration of contributions and supporting coherent decision-making. Together, these factors created the conditions for effective, multidisciplinary collaboration.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Methodological considerations<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Although a rigorous methodological approach was employed for this analysis, this qualitative interview study was limited to exploring a single team and its specific organisational setup within a healthcare setting among professionals from a specific PFC. For this kind of set-up, it was only possible to interview a smaller number of informants. Thus, it was not possible to confirm transferability to other teams in the Danish pelvic floor community because the organisational setup so far is different.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Furthermore, the findings of this study must be interpreted within the specific organisational context of a small-scale hospital characterised by a flat hierarchical structure. While the unique dynamics of this single-team setting provided rich, localised insights, these contextual factors may influence the direct transferability of the results to larger, more stratified clinical environments. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">As a strength of the study, we chose to involve outsiders to conduct interviews and analyse data, thereby strengthening data collection with an open mindset and without preconceptions. Further, informants could trust that the interviews were collected anonymously and did not conflict with later collaboration with their colleagues. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The interviewer was a novice in interviewing, which may have prevented her from elaborating on answers and providing information. This could have hidden underlying conflicts and challenges within the team and not given the full picture.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Conclusions<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">The effectiveness of moderator-guided multi-disciplinary consultations relies on both relational and organisational resources. Openness to collaborative processes and to the expertise of other professions emerged as key enablers of multidisciplinary teamwork within a specific PFC that engaged in multidisciplinary consultations. <\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Relational coordination through shared goals, shared knowledge, and mutual respect supported the team communication, decision-making and coherent patient pathways. The moderator emerged in this setting as a central structural and relational driver, facilitating the exchange of perspectives among relevant team members and collaboration, while promoting patient involvement. Effective moderation requires empathic, organisational, and coordination competencies, alongside the ability to sustain an overview of the consultation.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Team effectiveness developed gradually, underscoring the importance of time and adaptation in implementing multidisciplinary models. The study provides empirical insight into structuring collaboration in multidisciplinary care and offers a foundation for future research and implementation in comparable healthcare settings.
