C\u00f8liaki overses let, fordi sygdommen ikke altid pr\u00e6senterer klassiske mavetarm symptomer.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Et nyt case-baseret studie illustrerer, at diagnosen c\u00f8liaki ofte forsinkes i flere \u00e5r. Patienterne pr\u00e6senterer langvarige, uspecifikke symptomer s\u00e5som vedvarende tr\u00e6thed, v\u00e6gttab og mangel p\u00e5 mikron\u00e6ringsstoffer uden klassiske mavetarmsymptomer. Den diagnostiske forsinkelse f\u00f8rte til betydeligt nedsat livskvalitet, f\u00f8r korrekt diagnose og behandling blev iv\u00e6rksat. F\u00f8rst efter udredning for andre sygdomme som kr\u00e6ft eller<\/span><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> efter eget initiativ blev c\u00f8liaki p\u00e5vist, og symptomfrihed samt normaliseret ern\u00e6ringsstatus blev opn\u00e5et ved glutenfri kost.\u200b<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Cases<\/span><\/strong><\/p>\n<ul>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">En kvinde med over 10 \u00e5rs tr\u00e6thed, v\u00e6gttab og gentagne blodpr\u00f8ver viste mangel p\u00e5 jern, folat og D-vitamin. Hun blev f\u00f8rst udredt for kr\u00e6ft, f\u00f8r c\u00f8liaki blev diagnosticeret. Efter glutenfri kost normaliseredes blodpr\u00f8ver, v\u00e6gten steg, og symptomerne forsvandt.<\/span><\/li>\n<li><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">En tidligere praktiserende l\u00e6ge uden mavegener, men med ekstrem sult og lav D-vitamin. Han diagnosticerede sig selv i en sen alder efter at have l\u00e6st om c\u00f8liaki og fik bekr\u00e6ftet diagnosen med antistofpr\u00f8ver og biopsi. Efter glutenfri kost opdagede han, hvor meget bedre han kunne f\u00e5 det og v\u00e6gten steg.<\/span><\/li>\n<\/ul>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">\u00c5rsager til diagnostisk forsinkelse<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Forsinkelsen skyldes prim\u00e6rt, at c\u00f8liaki ofte gemmer sig bag uspecifikke symptomer, manglende klassiske tarmsymptomer og lav opm\u00e6rksomhed blandt b\u00e5de patienter og sundhedsprofessionelle. C\u00f8liaki kan ramme alle aldre og pr\u00e6senterer ofte med tr\u00e6thed, v\u00e6gttab og blodmangel p\u00e5 grund af malabsorption. Manglende viden og fokus p\u00e5 sygdommen hos praktiserende l\u00e6ger f\u00f8rer til fejltolkning og forsinkelse i relevant udredning, hvor kun f\u00e5 f\u00e5r udf\u00f8rt c\u00f8liaki-specifikke blodpr\u00f8ver i tide.\u200b<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Implikationer for klinisk praksis<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Det er vigtigt at f\u00e5 stillet diagnosen, fordi ubehandlet c\u00f8liaki kan f\u00f8re til \u00f8get risiko for kr\u00e6ft, knoglesk\u00f8rhed, blodmangel m.m. Ved mistanke om c\u00f8liaki tages en blodpr\u00f8ve (personen skal v\u00e6re p\u00e5 glutenholdig kost) med bestemmelse af c\u00f8liaki-antistoffer (IgA imod v\u00e6vs-transglutaminase). Blodpr\u00f8vetestning b\u00f8r ogs\u00e5 overvejes hos f\u00f8r-stegradssl\u00e6gtninge til patienter med c\u00f8liaki og patienter med visse andre autoimmune sygdomme. Tidlig diagnose og glutenfri kost kan hurtigt normalisere ern\u00e6ringsstatus og forbedre livskvalitet og forebygge komplikationer. \u00d8get opm\u00e6rksomhed blandt klinikere er afg\u00f8rende for
