Da visse parametre i calciumstofskiftet har kendt d\u00f8gnvariation, unders\u00f8ger dette studie, om naturligt lys p\u00e5virker calciumrelaterede biokemiske mark\u00f8rer hos patienter med behov for neurorehabilitering efter apopleksi.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Metode<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Dette studie er en sekund\u00e6r eksplorativ analyse af data fra et randomiseret kontrolleret studie om-handlende 45 strokepatienter kr\u00e6vende mindst 14 dages rehabilitering under indl\u00e6ggelse. Patienterne blev randomiseret til enten en interventionsenhed (IU) med naturligt d\u00f8gnrytmebaseret lys eller en kontrolenhed (CU) med standard indend\u00f8rs belysning. Blodpr\u00f8ver blev udtaget hver 4. time i 24 timer ved inklusion og udskrivelse. Der blev m\u00e5lt Calcium, Parathyroidea-hormon (PTH), Fosfat, Vitamin D og Basisk fosfatase. D\u00f8gnvariation blev analyseret via cosinor-analyse og variansanalyser.<\/p>\n<p><\/span><strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Resultater<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Calcium viste signifikant d\u00f8gnrytme i begge grupper b\u00e5de ved inklusion og udskrivelse og Fosfat udviste d\u00f8gnrytme ved udskrivelse i CU. \u00d8vrige parametre som PTH, Vitamin D og Basisk fosfatase viste ingen signifikant rytmicitet. Lyseksponering \u00e6ndrede ikke calciumrytmen. PTH-niveauet faldt signifikant i IU sammenlignet med CU (p = 0,019), men uden tegn p\u00e5 d\u00f8gnvariation.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Konklusion<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Studiet viste at calcium bibeholdt sin d\u00f8gnrytme efter stroke, mens \u00f8vrige calciumrelaterede parametre ikke gjorde. Naturligt lys havde ikke indflydelse p\u00e5 d\u00f8gnrytmen, men medf\u00f8rte et signifikant fald p\u00e5 67% i PTH-niveau. Dette tyder p\u00e5, at lys ikke er den prim\u00e6re regulator af d\u00f8gnvariation i calciumstofskiftet, men at f.eks. aktivitet, kost og kropsposition muligvis spiller en st\u00f8rre rolle.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Implikationer for praksis<br \/>\n<\/span><\/strong><span class=\"a_GcMg font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Selvom naturligt lys ikke syntes at p\u00e5virke d\u00f8gnrytmen i calciumstofskiftet, kan effekten af lys p\u00e5 PTH-niveauet potentielt mindske risikoen for knogletab hos immobiliserede patienter.
