At the Emergency Department at Nordsj\u00e6llands Hospital, local challenges became the starting point for a multifactorial organisational change, theoretically grounded in change management and organisational theory, using an iterative approach as a model for continuous development in practice.<\/p>\n<p class=\"my-2 [&amp;+p]:mt-4 [&amp;_strong:has(+br)]:inline-block [&amp;_strong:has(+br)]:pb-2\">One outcome was the development of a new competence profile \u2013 the acute care spot nurse \u2013 which has proved to be one of the concrete responses to challenges related to patient flow and resource utilisation. Crowding defineres overordnet som en situation, hvor behovet for akut behandling, overg\u00e5r de tilg\u00e6ngelige ressourcer i en akutafdeling (7).<\/span><\/p>\n<div class=\"farve-boks\" style=\"background-color: #b8e1c0; padding: 16px 32px; border-radius: 24px; color: black; margin-bottom: 32px;\">\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Crowding<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> konceptualiseres ofte med tre indbyrdes afh\u00e6ngige komponenter, der m\u00e5 medt\u00e6nkes, n\u00e5r man skal vurdere crowding:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">1.<\/span> <span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Input<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> i form af m\u00e6ngde og type af ydelse, der s\u00f8ges\/leveres i en akutafdeling.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">2.<\/span> <\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"><strong>Throughpu<\/strong>t<\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> som omhandler behandlings-processerne i en akutafdeling.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">3.<\/span> <span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Output<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\"> som omhandler flytning af patienter ud af afdelingen til en kontakt uden for akutafdelingen) (8).<\/span><\/p>\n<\/div>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Begrebet er s\u00e5ledes multifacetteret og kan manifestere sig i lange ventetider, overbe-l\u00e6gning, et presset arbejdsmilj\u00f8 for personalet, medarbejderudbr\u00e6ndthed, forringet patient-oplevelse, forv\u00e6rring af patienternes tilstand, lavere kvalitet i behandlingen, \u00f8get mortalitet og \u00f8gede \u00f8konomiske omkostninger for afdelingerne (9). <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">P\u00e5 akutafdelingen \u2013 Nordsj\u00e6llands Hospital (NOH) var problemet med crowding et stigende problem. Konkret udm\u00f8ntede det sig i utilstr\u00e6kkeligt patient-flow, flaskehalsdannelse og ineffektive arbejdsprocesser skabt af administrative byrder. Det p\u00e5virkede kvaliteten i behandlingen og arbejdsklimaet, der var pr\u00e6get af konflikter og et \u00f8get antal opsigelser i sygeplejegruppen. Der var d\u00e5rlig kommunikation mellem de ansatte og s\u00e5 manglende der fokus p\u00e5 kerneopgaven.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Baggrund<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Derfor gik vi i 2021 i gang med en omfattende organisatorisk udviklingsproces, hvor m\u00e5let b\u00e5de var at skabe en kulturel transformation i organisationen og efterf\u00f8lgende udvikle en organisations\u00e6ndring, der skulle sikre en mere effektiv og sammenh\u00e6ngende behandlings-proces, der kunne nedbringe overbel\u00e6gningen og im\u00f8deg\u00e5 crowding. <\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Derudover var m\u00e5let at \u00f8ge kvaliteten i behandlingen, sikre patient-flowet p\u00e5 hospitalet, \u00f8ge patienttilfredsheden, samt skabe et bedre arbejdsmilj\u00f8 for personalet og \u00f8get antallet af medarbejdere som blev i afdelingen.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Kulturelt udviklede afdelingen et nyt akut-koncept, der tog udgangspunkt i en sundhedsfaglig forankret tilgang til drift og strategisk ledelse, og med grunds\u00e6tningen, at det skal give mening for patienten. Den f\u00e6lles vision var at svare p\u00e5, om indl\u00e6ggelse af den konkrete patient er meningsgivende, og om de valgte interventioner giver v\u00e6rdi til patientens forl\u00f8b. Det blev kogt ned til sp\u00f8rgsm\u00e5let, som blev styrende for alle senere udviklingstiltag: \u201dEr hospitalet noget for patienten?\u201d (10). <\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Med udgangspunkt i den f\u00e6lles vision gik afdelingen i gang med en udviklingsproces, hvor afdelingen med kontinuerlig iterativ metode udviklede praksis over tid. Det resulterede i en multifacetteret organisations\u00e6ndring, Se <\/span><strong><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Figur 1<\/span><\/strong><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\"> for en visuel fremstilling af den tidligere og nye organisering.<\/span><\/p>\n<p><img data-recalc-dims=\"1\" decoding=\"async\" class=\"size-full wp-image-18469 aligncenter\" src=\"https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2025\/08\/FIGUR-1-1.png?resize=724%2C494&#038;quality=80&#038;ssl=1\" alt=\"\" width=\"724\" height=\"494\" srcset=\"https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2025\/08\/FIGUR-1-1.png?w=724&amp;quality=80&amp;ssl=1 724w, https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2025\/08\/FIGUR-1-1.png?resize=300%2C205&amp;quality=80&amp;ssl=1 300w, https:\/\/i0.wp.com\/sygeplejevidenskab.dk\/wp-content\/uploads\/2025\/08\/FIGUR-1-1.png?resize=600%2C409&amp;quality=80&amp;ssl=1 600w\" sizes=\"(max-width: 724px) 100vw, 724px\" \/><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Den nye organisering bygger p\u00e5 ruse princippet \u2013 at der kun er en indgang til hospitalet og \u00e9n vej igennem. Dette sikrer at ansvaret for de akutte patienter er entydigt og klart defineret (10). Afdelingen er blevet delt op i 2 hovedspor: <strong>Alfa<\/strong>, <strong>Beta<\/strong> og et <strong>specialistspor SUS<\/strong>. SUS er definerede patientgrupper og diagnoser, der sendes, \u201dsuser\u201d direkte til de fremskudte specialer, uden at patienten kommer ind og ligge i akutafdelingen. <strong>Alfa er det nye Fast Track<\/strong> eller <strong>High Flow spor<\/strong>. Her ses patienter som forventes at kunne udskrives igen, efter at der er taget stilling til deres problem \u2013 \u201dhospitalet er vurderet til ikke at v\u00e6re noget for dem\u201d, eller akutmedicinske speciale patienter. I dette spor er 56 % af pladserne stole, 22 % lejer og 22 % senge. <strong>Beta eller Low Flow sporet<\/strong>, er til patienter, \u201dhvor hospitalet er vurderet som noget for dem\u201d og indl\u00e6ggelse derfor er forventet. Samtidig er der gjort op med tidslige m\u00e5l og udviklet en organisering, der smidigg\u00f8r processerne og udnytter ventetiden, ved at levere indledende vurdering, blodpr\u00f8ver, eventuelt EKG og triage simultant i stedet for fortl\u00f8bende.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Centralt <\/span><\/strong><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">i denne nye organisering er den <strong>akutmedicinske spotsygeplejerske<\/strong>, hvis opgave det er at vurdere patienterne fra ventev\u00e6relset og tr\u00e6ffe klinisk beslutning om, hvilke spor patienten skal i, samt underst\u00f8tte de simultane processer s\u00e5 der ikke opst\u00e5r flaskehalse. <\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Den samlede organisations\u00e6ndring har haft stor effekt p\u00e5 effektiviteten og kvaliteten i afdelingen. Der er s\u00e5ledes set en reduktion af mere end 130 % overbel\u00e6gningen i akutafdelingen p\u00e5 84 %, fra 123,8 timer til 19,3 timer per kvartal, med samme patientindtag og uden \u00e6ndring i genindl\u00e6ggelser inden for 72 timer (11). Samtidig er medarbejdertilfredsheden \u00f8get, m\u00e5lt via trivselsm\u00e5ling fra 1.kvartal 2023 til 3. kvartal 2024, med stigning i b\u00e5de den generelle jobtilfredsheds og den faglige kvalitet. Den iterative tilgang bidrager til praksisudvikling ved at underst\u00f8tte en dynamisk og l\u00e6ringsbaseret udvikling, der tager h\u00f8jde for kompleksiteten i den kliniske hverdag og de involverede akt\u00f8rers behov (12). <\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Metoden var s\u00e6rligt velegnet fordi den gav mulighed for, i den kliniske kontekst, kontinuerligt at tilpasse udviklingen p\u00e5 baggrund af erfaringer og feedback. Samtidig var metoden afg\u00f8rende for at sikre kvalitet og kontinuitet i en kompleks proces (12).<\/span><\/p>\n<p><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Metoden var operationaliserbar i den komplekse klinisk hverdag, til at bev\u00e6ge sig fra det abstrakte til det konkrete, m\u00e5lbare eller h\u00e5ndgribelige, s\u00e5 den akutmedicinske spotsygeplejerskefunktion skridt for skridt kunne udvikles og implementeres i praksis.<\/span><\/p>\n<div class=\"farve-boks\" style=\"background-color: #b8e1c0; Som led i evalueringen af denne seneste udvikling, gennemf\u00f8rtes en kvalitativ tematisering af data fra LUP (Landsd\u00e6kkende Unders\u00f8gelse af Patientoplevelser) fra ultimo 2023. <\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Af den fremgik temaet:<\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\"><em> \u201dpatienter oplever at kommunikations- og informationsbehov ikke bliver m\u00f8dt\u201d<\/em>.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Evalueringen f\u00f8rte til en ny problemidentifikation:<br \/>\nHvordan sikrer vi, at alle akutmedicinske spotsygeplejersker kommunikerer, s\u00e5 patienterne oplever tilfredsstillende kommunikation p\u00e5 tv\u00e6rs af \u201dtid og belastninger\u201d? Afdelingen udviklede servicedesignet<strong> HAPS<\/strong> \u2013 <strong>H<\/strong>ej, <strong>A<\/strong>ktuelt, <strong>P<\/strong>lan, <strong>S<\/strong>ervice. Et kommunikationsprincip som skal sikre, at de akutmedicinske spotsygeplejersker p\u00e5 et hvilket som helst tidspunkt og i en hver situation, simpelt og systematisk kan tilrette kommunikationen til patienterne p\u00e5 en m\u00e5de som fremmer patienttilfredsheden.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Resultater<br \/>\n<\/span><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">Den akutmedicinske spotsygeplejerskes funktion, kompetenceprofil og uddannelsesforl\u00f8b<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-on font-feature-clig-on font-feature-calt-on text-decoration-none text-strikethrough-none\">At bestride denne nye centrale rolle i organiseringen i akutafdelingen kr\u00e6ver specifikke kompetencer, uddannelse og tr\u00e6ning. Denne opgave ligger hos oversygeplejerskerne og vagtplanl\u00e6gger, der tilpasser planerne efter aktuelle forhold og uforudsete \u00e6ndringer.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Diskussion<br \/>\n<\/span><\/strong><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Internationalt beskrives forl\u00e6nget liggetid og crowding i akutafdelinger, som den st\u00f8rste udfordring for patientsikkerheden i moderne akutbehandling (14). Studier viser, at crowding har en negativ effekt p\u00e5 akutsygeplejerskers arbejdsbyrde og arbejdsgl\u00e6de, og at sygeplejersker i akutafdelinger ofte oplever, at deres faglige ansvar udfordres, n\u00e5r de skal balancere v\u00e6sentlig sygepleje med en stor arbejdsbelastning (15).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">At opretholde et tilstr\u00e6kkeligt patientflow har l\u00e6nge v\u00e6ret en kerneopgave i akutsygeplejen og anses som vigtig af sygeplejerskerne selv. De oplever dog, at deres muligheder for at yde optimal pleje begr\u00e6nses, n\u00e5r de samtidig skal styre flowet \u2013 is\u00e6r i perioder med h\u00f8j patienttilgang. Dette kan f\u00f8re til manglende sygepleje (Missed Nursing Care) (16) og udg\u00f8r en trussel mod patientsikkerheden (15).<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Den nyudviklede funktion som akutmedicinsk spotsygeplejerske er et innovativt bud p\u00e5, hvordan denne udfordring kan tackles i en dansk kontekst. Fra sin centrale placering i organiseringen skaber den akutmedicinske spotsygeplejerske overblik i ventev\u00e6relset, allokerer patienter til den rette behandlingsvej og underst\u00f8tter et effektivt flow i afdelingen. Funktionen bidrager samtidig til at skabe ro for de \u00f8vrige sygeplejersker, som f\u00e5r frigjort tid og kapacitet til at fokusere p\u00e5 patientopgaverne, n\u00e5r de ikke ogs\u00e5 skal sikre overblik og flow p\u00e5 tv\u00e6rs af hele afdelingen.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Studier peger p\u00e5, at sundhedsv\u00e6senet i dag st\u00e5r overfor en stigende kompleksitet: flere patienter har komorbiditeter og komplekse tilstande, samtidig med at tilgangen af patienter stiger, og udstr\u00f8mningen reduceres pga. manglende sengekapacitet. Dette stiller \u00f8gede krav til sygeplejerskers evne til at prioritere og tr\u00e6ffe kliniske beslutninger (17), og h\u00f8j faglig kompetence blandt sygeplejersker er forbundet med bedre patientudfald og lavere d\u00f8delighed (18). Her spiller den lokalt forankrede uddannelse en afg\u00f8rende rolle i at sikre de akutmedicinske spotsygeplejerskers kliniske d\u00f8mmekraft og faglige niveau, samt i at rekruttere de rette profiler med de n\u00f8dvendige personlige og faglige kompetencer.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Perspektiverne for at udvikle funktionen som akutmedicinsk spotsygeplejerske er mange. Funktionen har allerede vist sig som et vigtigt redskab mod crowding, og indf\u00f8relsen af s\u00e5kaldt \u201cdobbelt spot\u201d \u2013 hvor to akutmedicinske spotsygeplejersker inds\u00e6ttes ved begyndende bel\u00e6gningsstigning \u2013 har givet afdelingen et fleksibelt v\u00e6rkt\u00f8j. Der arbejdes desuden p\u00e5 at videreudvikle funktionen ved at koble den til det AI-baserede redskab Praemostro, som allerede er implementeret p\u00e5 Odense Universitets Hospital. <\/span><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">Det AI- baserede redskab kan forudsige bel\u00e6gnings-stigninger, f\u00f8r crowding opst\u00e5r, og det forventes, at en tilpasning til akutmodtagelsen p\u00e5 NOH kan styrke pr\u00e6cisionen i vurderingen af behovet for for eksempel dobbelt spot og derved forbedre afdelingens beredskab.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-justify para-style-body\"><span class=\"OYPEnA font-feature-liga-off font-feature-clig-off font-feature-calt-off text-decoration-none text-strikethrough-none\">En anden udviklingsmulighed er at udvide den akutmedicinske spotsygeplejerskes rolle til ogs\u00e5 at omfatte delegerede rettigheder.
