Det antages, at modificeret v\u00e6ske og kost er nemmere at kontrollere og transportere i mund og sv\u00e6lg, s\u00e5 det bliver mere effektivt og sikkert at synke. Form\u00e5let med retningslinjen var at unders\u00f8ge, om modificeret v\u00e6ske og kost har st\u00f8rre effekt end vanlig behandling, og om modificeret v\u00e6ske og kost kan accepteres af patienter og sundhedsprofessionelle.<\/span><\/p>\n<h3 class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Centrale budskaber<\/span><\/h3>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">P\u00e5 baggrund af en systematisk litteraturs\u00f8gning og vurdering af kvaliteten af de identificerede studier samt en efterf\u00f8lgende h\u00f8ringsproces endte retningslinjen med f\u00f8lgende anbefalinger:<\/span><\/p>\n<h3 class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Modificeret v\u00e6ske &#8211; svag anbefaling<\/span><\/h3>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Anvend kun let, moderat eller meget fortykket v\u00e6ske efter omhyggelig unders\u00f8gelse og n\u00f8je overvejelse samt med personens informerede samtykke. Der er kun fundet studier, der inkluderer voksne personer med \u00f8vre dysfagi som f\u00f8lge af demens, Parkinsons sygdom eller hoved-hals-cancer.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">\u00c5rsagen til den svage anbefaling er, at den gavnlige effekt p\u00e5 lungebet\u00e6ndelse er usikker, samt at der er en tendens til v\u00e6gttab, dehydrering og ringe accept af den fortykkede v\u00e6ske.<\/span><\/p>\n<h3 class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">God praksisanbefaling<\/span><\/h3>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Det er god praksis at udf\u00f8re systematisk, klinisk udredning og benytte klinisk r\u00e6sonnering samt personinddragelse, n\u00e5r modificeret v\u00e6ske overvejes til personer med \u00f8vre dysfagi. Derudover b\u00f8r graden af dysfagi revurderes med passende mellemrum, s\u00e5ledes at v\u00e6skekonsistensen kan tilpasses den aktuelle situation.<\/span><\/p>\n<h3 class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Modificeret kost &#8211; god praksisanbefaling<\/span><\/h3>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Det er god praksis i samarbejde med patient\/p\u00e5r\u00f8rende at overveje at benytte modificeret kost til voksne personer med \u00f8vre dysfagi i den akutte fase, i genoptr\u00e6nings- og rehabiliteringsfasen, samt i den kroniske fase. Overvejelserne b\u00f8r omfatte diskussion af fordele og ulemper med m\u00e5lgruppen.<\/span><\/p>\n<h3 class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Indikatorer for implementering<\/span><\/h3>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Arbejdsgruppen peger p\u00e5, at f\u00f8lgende indikatorer vil kunne anvendes til at vurdere retningslinjens implementering:<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Under indl\u00e6ggelse (hospital, genoptr\u00e6ning og rehabilitering)<\/span><\/strong><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Indlagte patienter, der har \u00f8vre dysfagi, skal tilses af fagperson(er) med kvalifikationer\/kompetencer til at vurdere det rette niveau for kost- og drikkekonsistens og ern\u00e6ringsbehov mindst en gang ugentligt.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Indlagte patienter med \u00f8vre dysfagi skal involveres i beslutninger om niveau af kost- og drikkekonsistens, og det skal relateres til deres ern\u00e6rings- og v\u00e6skebehov, samt deres ern\u00e6ringstilstand. Der skal v\u00e6re et udvalg af forskellige smagsvarianter af kost- og drikkekonsistens, s\u00e5 chancen \u00f8ges ,for at patienterne kan v\u00e6lge efter deres smagspr\u00e6ferencer, hvortil muligheden for sufficient ern\u00e6rings- og v\u00e6skeindtag styrkes.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Kost- og drikkekonsistens skal dokumenteres i plejeforl\u00f8bsplan og genoptr\u00e6ningsplan, f.eks. i forbindelse med ern\u00e6ringsplaner.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">I eget hjem<\/span><\/strong><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Personer med \u00f8vre dysfagi, der udskrives til kommune og plejehjem, skal indenfor en uge tilses af fagperson(er) med kvalifikationer\/kompetencer til at vurdere det rette niveau for kost- og drikkekonsistens.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Personer med \u00f8vre dysfagi skal involveres i beslutninger om niveau af kost- og drikkekonsistens.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Ved f\u00f8rste bes\u00f8g skal der tages stilling til hyppigheden af opf\u00f8lgning med henblik p\u00e5 revurdering af funktionsevne.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Kost- og drikkekonsistens skal dokumenteres i d\u00f8gnplejeplanen eller i andet relevant dokumentationssystem.<\/span><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><strong><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">N\u00e6ste skridt<\/span><\/strong><\/p>\n<p class=\"cvGsUA direction-ltr align-start para-style-body\"><span class=\"OYPEnA text-decoration-none text-strikethrough-none\">Til optimering af retningslinjens implementering i klinisk praksis afholder arbejdsgruppen og CFKR et webinar i for\u00e5ret 2024, hvor retningslinjens hovedbudskaber og konsekvenser pr\u00e6senteres.
